APPENDIX A NOTICE OF VIOLATION Arkansas Power and Light Company Dockets: 50-313 Arkansas Nuclear One 50-368 Licenses: OPR-51 NPF-6 During an NRC inspection conducted on March 22-27, 1987, violations of NRC requirements were identified. The violations involved: (1) a failure to perform proper personnel breathing zone air sampling during work operations, and (2) a failure to comply with Technical Specifications for the monitoring of radioactive effluents from the Radioactive Waste Storage Building. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions, "10 CFR Part 2, Appendix C (1986), the violations are listed below:

1. Failure to Perform Breathing Zone Air Sampling 10 CFR Part 20.201(b) requires that each licensee shall make or cause to be made such surveys as: (1) may be necessary for the licensee to comply with the regulations in this part, and (2) are reasonable under the circumstances to evaluate the extent of radiation hazards that may be present. As defined in 10 CFR Part 20,201(a), " survey" means an evaluation of the radiation hazards incident to the production, use, release, disposal or presence of radioactive materials or other sources of radiation under a specific set of conditions.

Contrary to the above, the NRC inspector determined on March 24, 1987, that during the compaction of radioactive waste, the licensee failed to appropriately monitor personnel for exposure to airborne radioactivity.

This is a Severity Level IV violation. (Supplement IV) (313/8706-01; 368/8706-01)

2. Failure to Monitor Gaseous Effluents ANO Unit 2 Facility Technical Specification (TS) 3.3.3.9, " Radioactive Gaseous Effluent Monitoring," requires effluent monitoring instrumentation to be operable during gaseous releases from the Radioactive Waste Storage Building (RWSB), via the pathways set forth in paragraph 6 of Table 3.3-12 of the TS, to ensure that the radioactive effluent releases do not exceed the limits of TS 3.11.2.1. TS Table 3.3.12 Action Statements 31 and 32 require that when the continuous monitoring system is inoperable effluent releases may continue via this pathway provided samples are collected with auxiliary equipment.

Contrary to the above, the NRC inspector determined on March 24, 1987, that the auxiliary gaseous effluent sampling system installed and in use on the exhaust ventilation system of the RWSB, per TS 3.3.9, and Table 3.3-12 Action Statements 31 and 32 could not collect a 0706090293 B70601 f PDR ADOCK 05000313 G PDR L_. .

This is considered a Severity Level IV violation. (Supplement I)

Pursuant to the provisions of 10 CFR 2.201, Arkansas Power and Light Company is hereby requirad to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
