APPENDIX B N0f!CE OF DEVIATION Texas Utilities Electric Company Dockets: 50-445/86-31 50-446/86-25 Comanche Peak Steam Electric Station, Permits: CPPR-126 Units 1 and 2 CPPR-127 Based on the results of an NRC inspection conducted on November 1 through December 31, 1986, two deviations from commitments to the NRC were identified.

The deviations consisted of incorrect deletion of an inspection attribute from the inspection checklist by an Evaluation Research Corporation (ERC) inspector and invalidation of a deviation report (DR) by ERC engineering using a basis which was not consistent with Issue-Specific Action Plan (ISAP) VII.C criteria.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the deviations are listed below:

A. Paragraph 5.0.B.3.1 of ERC QI-046, Revision 2, states, in part, " Verify, by review of the inspection report or Construction Operation Traveler, that the bolts were tightened in accordance with the design drawings."

In deviation from the above, an NRC inspection of Verification Package R-S-STEL-117 involving member 02 on Drawing 2323-51-0564, Revision 2, revealed that the ERC inspector _had entered "N/A" (not applicable) for Attribute B.3.1, structural bolting tightness. Structural bolting tightness does, in fact, apply to eight 3/4" A193, Grade 88, Class 1 bolts associated with this member and tightness is addressed and signed off on Construction Operation Traveler CE-82-358-4802 (445/8631-0-02).

B. Paragraph 4.3.6.1 of Comanche Peak Response Team ISAP VII.c, Revision 1, requires that the ERC construction sample reinspection group review each DR for validity. A DR may be deemed to be invalid if the DR describes

. . . a condition which, in fact, is acceptable in accordance with the approved design or other appropriate data . . . ."

In deviation from the above, DR R-M-FFTA-001-DR6, which documents that i Chicago Bridge & Iron (CBI) did not supply a Metal Material Verification Summary Sheet (MVSS), was improperly invalidated even though the described condition is not acceptable. The CBI material requirement table, MRT-73121/2 requires, as a minimum, that the MVSS be supplied (445/8631-0-03; 446/8625-D-01).

Texas Utilities Electric Company is hereby requested to submit to this office within 30 days of the date of the letter transmitting this Notice, a written

! statement or explanation in reply, including for each deviation: (1) the reasons for the deviations if admitted, (2) the corrective steps which have

! been taken and the results achieved, (3) corrective steps which will be taken 5

2 to avoid further deviations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
