Inspection Report 50-293/87-17
A special radiological controls inspection was conducted by R. L. Nimitz of
this office on March 17-20, 1987 and April 7-10, 1987.
The finding of the
inspection were discussed with Mr. N. Brosse.
A telephone discussion was also held on April 17, 1987 between Messrs. Wiggins,
Shnbaky, and Nimitz of this office and Mr. K. Roberts and other members of
your staff regarding the findings of the inspection, particularly your
contamination control program as it applies to hot particle contamination.
The purpose of this inspection was to review the circumstances, your evalua-
tion and corrective actions associated with a March 16, 1987 personnel contami-
nation event. We also reviewed the implementation and adequacy of your: extrem-
ity monitoring program; exposure evaluation program; personnel contamination
control program; radiological goals program; and your exposure reporting.
Your immediate corrective actions to address the radiological safety concerns
associated with the personnel contaminction event were adequate. However
we did identify weaknesses in your staff's oversight and control of contamina-
tion. These inc'.uded unclear contamination control requirements contained in
radiation work permits, insufficient oversight of your protective clothing
laundry processing and lack of aggressive action on radiological occurrence
reports associated with personnel contamination incidents.
Based on the referenced telephone discussion, we understand that the Indepen-
dent Radiological Oversight Committee will be requested to perform a review of
this area and recommend improvements where needed.
If this is not your under-
standing, please contact this office.
We also identified weaknesses in your staff's ability to resolve previously
identified concerns reflecting an apparent lack of management oversight and
weakness in your system for task followup and accountability.
Concerns in
this area were discussed with your staff at the exit meeting.
No violations were identified. Your actions taken to address the weaknesses

discussed above, will be reviewed during a future inspection.
