Violation (482/8706-01): Fire Door Blocked Open

Finding:
Technical Specification (TS) 6.8.1 requires that, " Written procedures shall
be established, and maintained covering . . . a.
The applicable procedures
recommended in Appendix A of ~ Regulatory Guide (RG) 1.33,
Revision 2,
February 1978." Section 1.1 of Appendix A states that the fire protection
program should be covered by written procedures.
Licensee Administrative Procedure ADM 13-103, Revision 3,
" Fire Protection:
l
Impairment Control." has been established and implemented in accordance with
!-
Step 4.1 of ADM 13-103 requires "A Fire Protection Impairment Control Permit
l
form will be prepared for all fire protection system impairments . . ." . ."
Contrary to the above, on March 4, 1987, during a routine plant tour the NRC
,
inspector identified that TS Fire Door 34031 (control building elevation
2016 ft.) had been blocked open and unattended without the necessary fire
,
impairment permit form being issued.
Reason For Violation:

On March 4,1987, a Nuclear Regulatory Commission Resident Reactor Inspector
i
identified that Fire Door 34031 was blocked open by a hose but did not have
'a
Fire Impairment Control Permit.
The Control Room was notified of the
situation, and instructed that the door be closed.
Investigation revealed
.
that construction personnel had opened the door and placed a hose through
it while performing Work Request 723-87 without first obtaining
an
,
'
impairment
permit.
This event occured as a result of construction
personnel complying with previous verbal instructions from the
Fire
1
Protection CodPdinator that whenever the work activity requiring a barrier
l
to be breached, (e.g.
door 34031 to be blocked open), is estimated to last
i
less than one (1) hour,
a fire barrier impairment permit would not be
i
required.
Therefore the work activity on March 4,
1987 proceeded without
obtaining a fire impairment permit.

Corrective Steps Which Have Been Taken and Results Achieved:
Upon notification by the Resident Reactor Inspector that the door was
blocked open, the Control Room instructed that the door be closed.

Corrective Steps Which Will Be Taken To Avoid Further Violations

Construction craft personnel have been retrained on the requirements of the
administrative procedure governing Fire Protection Impairment Control and
'
verbally instructed to obtain a fire barrier impairment permit whenever a
fire barrier needs to be breached.
Additionally, the Superintendent of
,
Plant Support has instructed the Fire Protection Coordinator that a breach
of a fire barrier for any length of time requires a impairment permit.
