This transmits the September 1 - October 17, 1986 resident inspection findings by

Mr. T. Foley at the Calvert Cliffs Nuclear Power Plant, Lusby, Maryland.

Those

findings were based on observation of activities, interviews, and document reviews,

which have been discussed with Mr. Lemons of your staff.

Although I am aware of your extensive efforts aimed at repair of the gassing prob-

lem on #12 DG, I am concerned that these efforts have not been successful to date.

It appears that you have been unable to determine the root cause of combustion gas

getting into the jacket cooling water.

I have reviewed your letter to Dr. Murley

on this topic dated October 1, 1986 and the events surrounding this problem as

described in the enclosed inspection report.

It appears that your current intent

is to replace all parts that could provide a pathway for combustion gases to get

to the jacket cooling water system.

This methodology may prevent you from deter-

mining the root cause of this problem.

Please respond to this letter and in your

response, let me know of your plans with regard to preventing recurrence of this

problem.

An apparent violation of NRC requirements is noted within this report and is cate-

gorized in Appendix A under the revised NRC Enforcement Policy, 10 CFR 2 Appendix C.

You are required to respond to this letter and, in preparing your response,

you should follow the instructions in Appendix A to this letter.

Although corrective actions taken for this repetitive violation have been extensive,

additional examples persist.

10 CFR 2 Appendix C states that escalated enforcement

actions may be taken for repetitive violations of this severity.

A Baltimore Gas

and Electric and ' aC Management Conference was held at Region I on October 14, 1986,

to discuss the circumstances, corrective actions, an.' other actions taken to pre-

vent further recurrence.

In view of the extensive efforts already taken and the

additional actions being taken by BG&E management to try to preclude further occur-

rence plus the announced policy regarding disciplinary actions if further incidents

occur, no further actions by NRC are contemplated at this time.

Four reactor plant trips are noted within this report.

Three are similar to pre-

vious plant trips.

As stated in your Trip Reduction Task Force presentation to

the NRC on October 8,1986, we note that some of the recommendations resulting from

the task force efforts had the potential to avert several of these reactor trips.

We encourage you to expedite implementation of these efforts.

details of your security program that have been determined to be exempt from public

disclosure in accordance with 10 CFR 73.21 (Safeguards Information).
