r APPENDIX A NOTICE OF VIOLATION Rancho Seco Nuclear Generating Station Docket No. 50-312' 14440 Twin Cities Road Operating License No. DPR-54 Herald, California 95638-9799 As a result of the inspection conducted during.the period March 23 - April 3.

1987, a-violation of NRC requirements was identified. In accordance with the

" General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the following violation was identified:

A. 10 CFR Appendix "B" Criterion XV, " Nonconforming Materials, Parts, or Components" states:

" Measures shall be established to control materials, parts, or components which do not conform to requirements in order to prevent their inadvertent use or installation. These measures shall include as appropriate, procedures for identification, documentation, segregation, disposition, and notification to affected organizations. Nonconforming items shall be reviewed and accepted, rejected, repaired or reworked in accordance with documented procedures."

Quality Assurance Procedure QAP-17 " Nonconforming Material Control",

Revision 1, reads in part, " Systems, equipment and appurtenances, components, parts, or material which do not meet the specified requirements of purchase orders, design drawings, operational / test documents, or construction documents shall be considered nonconforming."

This procedure defines " Repair" as: "A disposition which pennits the reprocessing of material or change to the system to bring it into an acceptable condition, although still departing from established requirements. this disposition requires Engineering Review Board approval." This procedure specifies that the Engineering Review Board must approve all " Accept" or " Repair" dispositions of nonconforming material or systems. "The Board is composed of the managers of Quality Assurance, Nuclear Engineering, and Nuclear Operations or their designated representatives...."

Contrary to the above requirements, at the time of the Inservice Inspection of Reactor Coolant Pump (RCP) P-2108 on April 15, 1985, capscrews for the bearing positioner on the RCP were found broken. This nonconforming condition was not reported on an NCR and dispositioned accordingly. As a result the required management review and approval were not completed.

This is a Severity Level IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Sacramento Utility District is hereby required to submit a written statement or explanation to the U.S.

Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington DC 20555 with a copy to the Regional Administrator, Region V, and a copy to the NRC Resident Inspector, within thirty days of the date of the letter which transmitted this Notice. This reply should be clearly marked as a " Reply to a Notice of Violation" and should include: (1) The reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved; (3) corrective steps which will be taken to avoid further items of noncompliance; and, (4) the date when full compliance will be achieved. If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken. Consideration may be given to extending the response time for good cause shown.
