SUMMARY

- SEQUOYAH, DOCKET NOS. 50-327 AND 50-328 This refers to the management meeting conducted at our request at the Sequoyah

' site on October 23, 1986.

The meeting was held to discuss:

tracking and trending of out of-tolerance conditions associated with Technical Specification I

(TS) and compliance instruments; the escalation and resolution of Quality Assurance audit findings; the numerous inadvertent starts of the emergency diesel generators; and restart plans.

A summary of the meeting topics is provided in Enclosure 1.

A list of attendees at the meeting is shown in.

It is our opinion that this meeting was beneficial in that we feel it provided both the NRC and TVA a better understanding of the issues.

As acreed in the meeting, we are expecting submittals from you addressing: your plans to review past out-of-tolerance occurrences to ensure reliability of all TS equipment; the update of Sequoyah's drawings; and your review plan to assess Sequoyah's operational readiness.

In accordance with Section 2.790 of NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this letter and its enclosures will be placed in the NRC Public Document Room.

Should you have any questions concerning these matters, we will be pleased to discuss them.

SUMMARY

On October 29, 1986, representatives of Tennessee Valley Authority (TVA) met with the NRC, at NRC's request, at the Sequoyah site.

The topics of discussion were tracking and trending of out-of-tolerance conditions associated with Technical Specification (TS) and compliance instruments; the escalation and resolution of Quality Assurance (QA) audit findings; the numerous inadvertent starts of the emergency diesel generators (EDGs); and restart plans.

Acknowledging that the licensee's newly implemented tracking and trending program appeared to be adequate to ensure future out-of-tolerance conditions are captured and reviewed for root cause determination, NRC requested that the licensee address their plans to review past out-of-tolerance occurrences to ensure reliability of all TS equipment.

The licensee indicated that they plan to review all potentially reportable occurrences (PR0s) since plant licensing (in lieu of 1984 as stated in the nuclear performance plan).

Interviews of lead engineers are to be conducted to validate the PR0 data base.

These engineers are to draw upon personal files, old logs and system files.

The licensee committed to make a submittal describing the details of this plan and to follow it up with the subsequent results.

In light of repeated stroke time failures of upper head injection isolation valves, the licensee indicated that they will now be making use of the set screws on the associated hydraulic lock valves as well as seeking a TS change to increase the acceptable stroke time tolerance.

In regard to TVA's apparent deficiency in promptly escalating and resolving quality assurance audit findings, the licensee indicated that they are presently re evaluating their goals and objectives for adequacy and timeliness to prevent the necessity for escalation.

In conjunction with this, the licensee states that corrective action report (CAR) meetings are now being held to assure the appropriate pecple understand the problems prior to the issuance of the CAR.

A submittal addressing the planned corrective actions will be made in response to violation 86-53-01.

With respect to the numerous inadvertent EDG starts, NRC expressed concerns over the unnecessary safety system challenge being imposed, as well as the indicated existence of long standing issues such as configuration control, drawing control and modifications.

The licensee stated that the majority of the inadvertent starts were due to personnel error during their ongoing fuse change out program.

They indicated that the personnel errors were not related in any way, and that they felt that personnel performing the fuse change out were adequately trained.

Being sensitized to the problem, the licensee stated that where possible, they plan to make the associated EDG inoperable when changing fuses or performing work in related electrical panels and boards.

Regarding the inadvertent EDG start event that occurred on September 14, 1986, as a result of inadequate drawing control, the licensee indicated that prior to restart, they will combine the information from selected Unit 1 and Unit 2 electrical prints that have shared equipment.

In addition, the control room prints will also be changed before restart to show only the "as constructed" configuration in lieu of a marked up "as designed" configuration.

On request, the licensee agreed to make a submittal addressing all plans and commitments relating to the drawing upgrade at Sequoyah.

The meeting was closed with a general discussion of restart plans with respect to NRC involvement and possible hold points.

TVA stated that they were developing an operational readiness review plan, and would submit it once it was approved.

Both TVA and NRC agreed to meet again and talk specifics once each other s restart plans have been firmed up.
