APPENDIX A NOTICE OF VIOLATION Omaha Public Power District Docket: 50-285/86-34 Fort Calhoun Station License: DPR-40 ,

I During an NRC inspection conducted on December 1-31, 1986, violations of the NRC requirements were identified. The violations involved failure to follow procedures when using the waste gas sampling system, failure to install fire barrier / security doors in accordance with documented installation instructions, and failure to post the auxiliary building as a radioactive materials area. In accordance with the " General Statement of Polic Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (y and1986),the violations are listed below:

A. Technical Specification 5.8.1 requires that procedures, as listed in Appendix A to Regulatory Guide 1.33, be established and implemented.

Section 7.C of Appendix A to Regulatory Guide 1.33 includes procedures j- related to sampling and monitoring of the gaseous effluent system.

Procedure OI-WDG-3 has been established in accordance with this Technical Specification requirement and requires that the waste gas sampling system will be placed in a standby mode upon completion of sampling.

i Contrary to the above, the waste gas sampling system was found, on three occasions on December 4, 29, and 31,1986, during this inspection period, not to have been placed in the specified standby mode after completion of sampling.

B.. Criterion V of Appendix B to 10 CFR Part 50 and Section A.6 of the i licensee's quality assurance plan require that activities affecting

! quality be described in documented instructions, installed in accordance with the instructions, and appropriate quantitative acceptance criteria be provided.

Installation package MR-FC-85-38 A/B was established-in accordance with the requirements specifying that the installation requirements for the ,

fire barrier / security doors (1025-4,1011-28, and 1036-2) be installed in '

accordance with the Underwriter Laboratory (UL) standards.

Contrary to the above, fire barrier / security doors were not installed in accordance with documented instructions and the instructions did not provide appropriate quantitative acceptance criteria as related to the UL

standards, in that:

i l 1. Work proceeded past the specified QC hold points during installation of doors 1036-2 and 1011-28.

2. The installation-packages for doors 1025-4, 1036-2, and 1011-28 were signed off as " work complete," when in fact steps were not complete.
3. The installation package step for painting door 1025-4 was signed off as " work complete;" however, door 1025-4 was not painted.
4. The gap at the bottom of door 1025-4 was excessive.
5. Six holes were left unplugged in door 1036-2, which exposed the door Core.

This is a Severity Level IV violation. (Supplement I.D) (285/8634-02)

C. Section 20.203(e) of 10 CFR Part 20 and Section 3.0 of the licensee's radiation protection manual require that each radioactive materials area be conspicuously posted with a sign bearing a radiation caution symbol and the words, " Caution-Radioactive Materials Area". The licensee has previously established that the auxiliary building is a radioactive materials area.

Contrary to the above, the licensee failed to post a sign at all entrances to the auxiliary building to identify the auxiliary building as a radioactive materials area.

This is a Severity Level V violation. (SupplementI.E)(285/8634-01)

This inspection indicated that actions had been taken by the licensee to correct apparent Violation C and to prevent recurrence. Consequently, no reply to Violation C is required and we have no further questions regarding this matter.

, Pursuant to the provisions of 10 CFR Part 2.201, Omaha Public Power District is l hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violations if admitted, (2) the corrective steps which have been taken and the results achieved, i (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
