50-400/86-76 Reported Violation:
Technical Specification 6.8.1 states that written procedures shall be established, implemented, and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978. Appendix A of Regulatory Guide _1.33, Revision 2, February 1978 recommends that procedures should be written for each surveillance test. Operational Surveillance Test Procedure, OST-1085, lA-SA Diesel Generator Operability Test, Step 30 directs to, locally at the ECP, monitor individual cylinder exhaust temperatures on Temperature Selector Switch. When all cylinder exhaust temperatures are less than 500*F, stop the Emergency Diesel Generator.
Contrary to the above, the Licensee failed to properly implement procedure OST-1085, in that individual cylinder exhaust temperatures were not monitored prior to securing Emergency Diesel Generator lA-SA on November 6, 1986.
This is a Severity Level IV violation (Supplement I).
Denial or Admission and Reason for the Violation:
The violation is admitted as stated. The violation occurred during performance of OST-1085 "lA-SA Diesel Generator Operability Test". This procedure is used to verify acceptable start time and load carrying capability. Upon initial starting of the Emergency Diesel Generator (EDC) 1A-SA, it was discovered by the operator that the start time had not been determined. This was discovered before the EDC had been loaded. The operator notified the Shift Foreman and was instructed to secure the EDC and restart the test. The operator stopped the EDC and began preparations to reset the diesel and to restart the test.
CP&L finds that an inadequacy in the procedure is the reason for the violation. OST-1085 failed to provide instructions to the operator on securing the EDC if the OST was stopped prior to completion of the test.
Corrective Steps Taken and Results Achieved:
Normat shutdown sequence is to idle the EDC under no load until the exhaust temperatures are below 500*F.
During the subject incident, the EDC had not been loaded and had only been idling for a short time. Even though the exhaust temperatures were not verified by the operator per Step 30 of OST-1085, the temperatures would have been less than 500'F.
Thus, no damage to the EDC could have resulted from this incident.

A note is being added to the EDC OST's (including OST-1085) requiring the operator to refer to the applicable steps in the OST for securing the engine if the EDG is to be stopped prior to completion of the test.

- The applicable OST's will be revised adding the note as stated above, by February 12, 1986.
