HIGHLIGHTS OF WEEKLY INFORMAT10?i REPORT WEEK ENDING AUGUST 22, 1986 Perr.y 1/ Davis-Besse The staff was advised by the Cleveland Electric Illuminating Company that the Governor of Ohio (Richard F. Celeste) will hold a Press Conference August 15, 1S85 at 2:30 p.m. EDT to announce the States' withdrawal of its approval of the Perry emergency evacuation plans. The Governor's action is being taken in light of recent sobering incidents both here and abroad -- the explosion and fire at the Chernobyl power plant and the earthquake that occurred early

.this year near the Perry plant. "Both incidents have raised many legitimate concerns about the adequacy of evacuation plans at the Perry and Day 1s-Besse plants " the Governor said. The Governor indicated his intention to establish a Connission to investigate concerns raised on Ohio with regard to the adequacy of the Perry hnd Davis-Besse EPZ in the aftermath of the Chernobyl accident; and assure that the lessons learned from Chernobyl are appropriately considered -

in the Perry emergency plans.

River Bend Station Gulf States Utilities has elected to place River Bend Station into an outage i

to conduct tests on its General Electric supplied turbine and generator.

River Bend Station will begin its shutdown during the evening cf August 15,

' 1986. General Electric recommended that the licensee conduct these tests after GE's analysis of extensive damage to a similar GE supplied turbine and generator caused by high vibration at a nuclear power plant in Taiwan in July, 1985. Gulf States Utilities expects the outage to last twelve days.

Sheffield Site The State of Illinois and U.S. Ecology, the operator of the Sheffield Low-l Level Waste Site, have decided to try to resclve outstanding issues concerning l closure before Illinois becomes an agreement state (scheduled for January 1, 1987).

l The State, U.S. Ecology and the NRC staff met with the ASLB in a prehearing a conference on August 19, 1986. The purpose of the conference was to 1) relate which closure issues remain to be resolved, 2) present schedules for a hearing on unresolved closure issues, and 3) discuss legal issues.

l LLRWPAA NRC staff were interviewed on the NRC's role on the implementation of the Low-Level Radioactive Waste Policy Amendments Act of 1985 (LLRWPAA). The interview, for a national broadcast, was conducted by the Media Monitor Radio network which is an affiliate of the " Christian Science Mcnitor." Staff explained the basic principles of the Low-Level Act of 1980 and the access and milestone requirements of the 1985 Amendments Act.

H. B. Robinson Steam Electric Plant Unit No. 2 A 10 CFR Part 72 license was issued on Wednesday August 13, for dry spent fuel storage at the H. B. Robinson-2 plant site. Only fuel that has cooled a minimum of 5 years will be stored. The fuel would be stored in dry shielded canisters which are stored in concrete storage modules. Each concrete module

Arkansas Nuclear One. Units 1 & 2 Region IV has reported on August 12, 1986, that an emergency diesel generator for ANO-2. failed its 18 month operability load test near the end of the 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> run when it had to be shutdown due to high cylinder differential temperatures on June 6, 1986. At the time Unit 2 was and is in its current refueling outage. The'cause of the failure was fuel starvation due to a clogged fuel strainer in the line from the diesel fuel day tank. The licensee subsequently

{ found sludge, grit or sediment in both diesel generator day tanks. Some i biologi. cal growth was also four.d in the sediment samples. Based on the sediment sample analysis results, the licensee believes that the sediment buildup was t

. caused by aging of the fuel oil and bacterial action. The licensee has not determine the cause of the grit found in the samples yet.

The licensee's corrective action to date has been to clean all the Unit 2 day tanks and storage tanks and to recirculate the fuel oil in the underground i

tanks though a 5 micron filter to the above ground storage tanks. The licensee stated that the current Technical Specification sampling program had not provided any indication of this problem before this occurrence. Unit 2 Technical

Specifications require a 92 day sampling of the storage tanks. Unit 1 Technical Specifications require a monthly sampling cf the fuel storage tanks. Unit 1 day tanks were sampled from the bottom of the day tank and no problems were found.

Catawba Unit 2 I

, On August 19; 19"6, Catawba Unit 2 entered comercial operation following completion of the 100-hour warranty run at 100% power on August 18, 1986.

l Catawba Unit 2 received a low-power operating license on February 24, 1986.

The Unit achieved criticality on May 8 and received its full power operating

license on May 15. Its first electricity was generated on May 18.

The staff stated in their filing on scheduling matters dated august 4, 1986,

that " CASE's acceptance of Applicants' plan to litigate the adequacy of the
CPRT process separately from the adequacy of program implementation, as originally proposed by Applicants Manage-ment Plan in June 1985, comes to late, since the prime reason for such a process was to expedite litigation by addressing the CPRT Program Plan during the time period prior to implementation and ,

completion.

Braidwood 1 On August 18, 1986, Commonwealth Edison, pursuant to 10 CFR 50.57(c), filed a motion for authorization of. fuel loading and precritical testing of Braidwood 1.

'The motion states that a boron concentration of 2000 ppm will be maintained in the reactor coolant system, and to preclude inadvertent boron dilution, nonborated water sources will be isolated by mechanically locking closed the appropriate valves.

The staff has 15 days to respond to the motion, unless either the intervenor or staff request a delay.

Waterford 3 On August 15, 1986, Louisiana Power and Light Company announced that they have chosen Jerrold G. Dewease to replace R.S. (Mike) Leddick, as Sr. Vice President for Nuclear Operations. Mr. Leddick left LP&L in March of this year to assume a management position with the City of Minneapolis. Mr. Dewease, an electric utility executive with 26 years of experience, served as Vice Fresident of Nuclear Plant Operations for Houston Lighting and Power Company prior to joining LP&L.

NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT FOR AUGUST 18 - 24, 1986

1. DEFUELING

- Bulk defueling operation continues using heavy duty hydraulic chisel and small spade bucket tools. Two fuel canisters have been filled to date since operations began August 12, 1986.

- Hydrogen peroxide was added to reactor coolant to kill the microorganisms which reduce visibility, thereby hampering defueling operations.

2. PLANT STATUS

. - The reactor remains in long term cold shutdown, vented to the atmosphere. Core cooling is by natural heat loss to ambient building atmosphere. The average incore thermocouple reading is 82 F.

- The airborne radioactivity on the defueling platform above the reactor vessel is about 6.0 E-7 uCi/cc Tritium and 5.6 E-12 uCi/cc particulates, predominately Cesium-137 and Strontium-90. About 15 feet of water over the core region and 6 feet over the carousel holding the defueling canisters provide shielding.

3. ENVIRONMENTAL MONITORING

- US Environmental Protection Agency (EPA) sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and The City of Lancaster Agreement.

- The Lancaster water sample taken at the water works river intake and analyzed by EPA consisted of a seven day composite sample taken August 3 - 9, 1986. A gamma scan detected no reactor related activity.

- TMI water samples taken by EPA at the plant discharge (includes Units 1 and 2) to the river consisted of seven daily composite samples taken from AugJst 2 - 9, 1986. A gamma scan detected no reactur related activity.

- The EPA analysis of the NRC outdoor air sample for the period August 14 - 21, 1986 showed that concentrations of Cs-137 and I-131 were below the lower limit of ouantitative detectability for the system.

4. AUXILIARY AND FUEL HANDLING BUILDING ACTIVITIES,

- Hands-on decontamination of Neutralizer Tank Pump room and Reactor Coolant Evaporator room has been started.

- Kelly vacuuming decontamination on "A" Bleed Tank room and Concentrated Liquid Waste Pump .com continues.

, Items of Interest Week Ending August 22, 1986 General Electric Morris Operation A byproduct material license was issued pursuant to 10 CFR Part 30 authorizing General Electric (GE) to use facilities at its Morris, Illinois site to con-duct small scale tests and studies of the GE Aztech process for dewatering and solidifying reactor resin slurries. The tests ano demonstrations, invol-ving not more than 10 curies of activity contained in 10 liter samples of reactor resins, will be conducted in laboratories and shielded cells presently used la connection with the receipt and storage of spent reactor fuel at the Morris Operation under a 10 CFR Part 72 license. The GE Aztech process is being marketed by the firm for use at reactor sites to treat and solidify

, reactor resins using a polyester material in preparation for disposal.

Status of' Hearings

1. Sequoyah fuels, Gore, Oklahoma, UF to UF 4 6

By memorandum and order dated July 23, 1986, the conference of parties to the informal hearing scheduled for August 11 was delayed until a date yet to be determined in September. The dates for the informal hearing have also been delayed pending Commission decisions regarding institution of additional rules for the hearing.

2. Sequoyah Fuels, Gore, ' Oklahoma, Comprehensive Waste Disposal Plan The hearing is being held in abeyance pending submittal of a new plan as
part of the revision to the decommissioning plan required by Source Material License No. SUB-1010, Condition No. 21.
3. Kerr-McGee, West Chicago, Illinois, Kress Creek Deco.1tamination The ASLB decision dated June 19, 1986, dismissed the Order to Show Cause issued *.o Kerr-McGee Chemical Corporation on March 2, 1984. On August 11, 1986, the staff filed'a notice of appeal with ASLAB.
4. Kerr-McGee Rare Earths Facility, Decommissioning Plan, West Chicago, Illinois This hearing is being held in abeyance pending the staff's completion of the supplement to the Environmental Impact Statement. The current schedule calls for issuance of the Draft Supplement by October 1, 1986, and the Final by May 1, 1987. The Board has indicated that they may resume a portion of the Hearing after the Draft is issued.

Mixed Waste A Commission paper was submitted to E00 in 8/19 providing a status report on NRC/ EPA interactions. The paper notes that E?A will not be able to issue or describe their location standards on a timetable consistent with that of the LLRWPAA. However, EPA has agreed to work closely with NRC in trying to resolve location standards and other issues related to mixed waste.

UMTRA Program DOE States / Tribes Meetina On August 26 and 27, 1986, NRC staff will attend a States / Tribes meeting conducted by DOE in Durango, Colorado. The meeting will serve as a forum for presentation and discussion of the latest developments in, and progress of, the Uranium Mill Tailings Remedial Action Project (UMTRAP). In addition, visits to two of the UMTRAP sites, Durango and Shiprock, will be conducted.

Safeguards Regulatory Effectiveness Review (RER) Scheduled for Indian Point 3 The NHSS safeguards staff, assisted by U.S. Army Special Forces personnel, is scheduled to conduct a Safeguards RER at Indian Point 3 from September 8-12, 1986.

c. Operations Center (1) A FEMA emergency preparedness exercise report for Limerick was forwarded to the region for appropriate action. There were no deficiencies in ottsite preparedness observed during the exercise.

(2) FEMA has reported that the alert and notification system for the Hatch plant is adequate to promptly alert and notify the public in the event of a radiological emergency. FEMA's report is based on an engineering design review and the results of a telephone survey.

d. Emergency Preparedness A member of the Emergency Preparedness Branch supported the regional inspection team at Palisades in the evaluation of the licensee's performance during the annual exercise on August 19, 1986.
e. Vendor Program (1) A member of the Quality Assurance Branch met this week with NMSS staff to discuss the methodology used in design inspection which may be of use in their review of the High Level Waste Repository Program.

(2) A member of the Quality Assurance Branch is in Region II this week conducting orientation training on the temporary instruc-tion for performance-oriented QA inspections.

1. Enforcement Conference Radiation Technolocy, Inc. - an enforcement conference was held on August 12, 1986. The conditions for lifting the suspension and acting on the license renewal appl.ication were discussed. RTI subsequently accepted the conditions in a written response dated August 15, 1986 and the staff expects to lift the suspension and issue the renewed license by August 22, 1986.

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending August 22, 1986 International Code Assessment and Applications Program (ICAP)

The purpose of the International Code Assessment and Applications Program (ICAP) is to obtain an independent assessment of uncertainties of the NRC's thermal hydraulic code developed for LOCA and systems transients. Currently RES has 11 offective agreements with European and Asian countries, and 3 proposed agreements are under negoti3 tion.

During the week of August 11, 1986, the RES staff and supporting contractors (LANL, SANDIA and INEL) held meetings to discuss the uncertainty analysis methodology, The method will be used to quantify code uncertainties in calculating reactor transients. The code uncertainties will be determined from the differences between code calculations and the test data. The ICAP participants will provide NRC with code calculations and test data while the NRC will perform the uncertainty studies.

Three methods were discussed: (1) the Conservative method which would calculate the uncertainty in a key parameter; e.g., peak clad temperature (PCT) by adding the maximum bias ( A max) during the transient and the maximum experimental variability (2dexp); (2) the Statistical Method which would require a sufficient large data base that has a sufficient mix of transients and facility representations; and (3) the Bias Method which would require the analysis of all the biases for different scales, parameter facilities, etc.

As a result of discussions, the parties agreed to select the Statistical Method.

The method will be presented to a panel of experts in late September,1986 for critique and recommendations.

A matrix of assessment tes,ts for the ICAP was also discussed. It was agreed that a complete test matrix must be developed to ensure that proper tests are performed at each type of facility to meet requirements of the uncertainty analysis. At the present each ICAP participant has selected some assessment tests to perform. After a complete test matrix is developed, the NRC will suggest each participant to perform additional tests and/or calculations.

The parties also agreed that the NRC publish an ICAP Annual Report to document the status of the program. The report will summarize the ICAP experimental tests and calculations completed, the uncertainty analysis of the results and their regulatory significance, completed major milestones. RES intends to publish this report in March of every year for the duration of the program.

2 Severe Fuel Damage Test and NRU Reactor The test assembly for the NRU severe fuel damage reactor test FLHT-4 was loaded into the reactor on Tuesday, August 19 for the August 25 planned nuclear heatup, In this test an eleven rod full length assembly of commercial enrichment fuel rods will be nuclear heated to 2400K and above. The assembly will be held at power for an additional 20 to 30 minutes after reaching 2400K to establish hydrogen generation rates during and after cladding relocation. One of the test fuel rods was commercially irradiated to a nominal 30 GWD/MT and fission product release and transport will be monitored during and after nuclear heatup.

Senseney of IP to discuss notification and coordination between the NRC and the AECB during emergencies at facilities situated along the U.S./ Canadian border. On Thursday he was scheduled for meetings at the Federal Emergency Management Agency.

On Thursday Dr. Sandor Kovacs, Head of the Environmental Section of the Israeli Electric Corporation, met with D. Cleary and C. Hickey, SOEI/DSR0/NRR, to discuss environmental siting policy.

0FFICE FOR ANALYSIS AND EVALUATION OF OPERATIONAL DATA ITEMS OF INTEREST Week Ending August 22, 1986

1. AE0D issued AE0D/P603, a report of a trends and patterns analysis of engineered safety feature (ESF) actuations which occurred during the last six months of 1984 at copinercial U.S. nuclear power plants. The investigation documented in this report was limited to those ESF actuations which involved systems other than the reactor protection system (RPS). RPS actuations were the subject of a companion study, AE0D/P504.

Based on the analysis and evaluation of these actuations, it was apparent that the majority of the ESF actuations were unnecessary and that the rate of these actuations could be decreased by (a) reducing the number of equipment failures during normal operation, (b) reducing the numbe'r of personnel errors during maintenance and testing, and (c) reducing spurious actuations due to electrical noise spikes.

2. AE0D issued AE0D/P604, a systematic review of the operating experience data bases with a focus on "new plants." For these 19 newer units, trends and patterns, as well as plant-by-plant comparisons were prepared for RPS actuations, ESF actuations, security events and miscellaneous events. Other potential trends and patterns were also evaluated such as the frequency of entering LC0's, evente attributable to testing, maintenance, or operations, and trends in the NPRDS data base for the failure of components at new plants. However, this data did not support significant conclusions.

The goal of our new plant study is to isolate specific trouble areas for these new plants and allow us to focus on the causes of these trouble areas. This study has produced two results in that vein. First, it identified certain plants and plant characteristics that appear to have been outliers during the period studied. These will be further examined to gain an understanding of the root causes for the events. Secondly, the study indicates that a certain predictive ability does exist for new plants. That is, if ~a high trend develops in the first two months of operation, it is likely that the plant will be an outlier in the absen'e c of some uncharacteristic corrective action. The further analyses to identify root causes for these events at new plants are the subject of other AE0D studies now in progress.

3. AE0D Case Study C602, " Operational Experience Involving Turbine Overspeed Trips" was issued on August 20, 1986. The study was performed in response the action item 8(f) of the actions di#ected by the ED0 to respond to the NRC staff investigation of the Jur.e 9,1985 event at Davis-Besse. The study revised and evaluated past operating experience involving overspeed trips of PWR auxiliary feedwater (AFW) turbine-driven pumps. In addition, the study was extended to include a review of August 22, 1986 Enclosure J

turbine overspeed trip operating experience on BWR high pressure coolant injection (HPCI) and reactor core isolation cooling (RCIC) turbine-driven pumps. A total of 128 events . involving overspeed of steam turbines associated with AFW, HPCI and RCIC systems were reviewed. These events occurred from January 1972 to September 1985 and were identified primarily from Licensee Event Reports (LERs).

Review of these events indicates that the dominant attributed causes of AFW turbine overspeed trips are speed control problems associated with governors, and trip and reset problems associated with trip valves and overspeed trip mechanisms. These problems are primarily the result of inadequate performance by plant personnel, inadequate procedures, and '

in. sufficient system design considerations. The governor speed control problems are (1) slow response of the Woodward Model PG-PL governor, (2) entrapped oil in the speed setting cylinder of the Woodward Model PG-PL governor, (3) incorrect governor setting and (4) water induction into the turbine. The trip and reset problems stem from the complexity of reset operations and a lack of trip position indication.

The study provides several recommended corrective actions aimed at addressing the identified causes for turbine overspeed trips. The study and recommendations were forwarded to NRR and IE for inclusion in their respective ongoing progr6ns to improve AFW reliability.

Also provide any information available on the number of desk audits associated with promotion of women and minorities as compared with thene same numbers for whit.e males. Staff may limit its study to the past 3 years.

n. With respect to the staff's initiatives on developing criteria for judging manager's EEO performance (reference memo of May 14, 1986), please provide answers for the following questions:
1. Do those criteria apply to all managers, not just Office Directors and Regional Administrators?
2. If not, what criteria are applied to lower level managers?
3. What steps are taken to ensure that all managers are aware of those criteria which apply to them?
