NOTICE OF VIOLATION Illinois Power Company License No. CPPR-137 As a result of the inspection conducted on July 1-28, 1986, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violation was identified:
10 CFR 50, Appendix B Criterion V, as implemented by the Illinois Power Company Operational Quality Assurance Manual (0QAM) requires that activities affecting quality shall be accomplished in accordance with written instructions, procedures, and drawings.
Procedure CPS No. 1029.01, Revision 10, Preparation and Routing of Maintenance Work Request (MWRs), requires in Paragraph 8.2.21 that the Maintenance Group Supervisor evaluate the MWR for adequacy of corrective action taken and work completion.

Cor.trary to the above, activities affecting quality were not accomplished in accordance with CPS No. 1029.01 in that MWR C06466 was signed off as completed by the maintenance technician and the maintenance supervisor when the work required by the MWR had not been completed. This was a repeat of a previous violation.
This is a Severity Level IV violation (Supplement II).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation:
(1)correctiveaction taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when fullscompliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
