.- ~ g 4 a APPENDIX A NOTICE OF VIOLATION Southern California Edison Company Docket No. 50-361, 50-362 San Onofre Units 2 and 3 License No. NPF-10, NPF-15 As a result of the inspection conducted during the period of November 15 through December 20, 1986, a violation of NRC requirements was identified. In accordance with the General Statement of Policy and Procedure for NRC Enforcement Actions, 10 CFR Part 2 Appendix C, as revised, 49 FR 8583 (March 8, 1984), the violation is set forth below:

Technical Specification 6.8.1.c requires that written procedures be established, implemented and maintained covering surveillance and test activities of safety related equipment.

Technical Specification 4.3.3.8.1 requires that radioactive liquid effluent monitor 2/3 RT-7813 be demonstrated operable by periodic performance of channel calibrations and functional tests.

Station procedure 5023-XXV-9.343 (Radiation Monitor Isotopic Calibration) implements the Technical Specification surveillance calibration requirement for monitor 2/3 RT-7813. Section 6.5.1.1.9 of this procedure ensures that the monitor is properly returned to service following calibration and requires that "as left" valve positions be recorded on the valve position verification sheet, attachment 4 of the procedure.

Station' procedure 5023-II-4.16 (Radiation Monitor Channel Functional Test) implements the Technical Specification surveillance functional test requirement for monitor 2/3 RT-7813. Section 3.14 of this procedure requires that all specified prerequisites be verified prior to performance of work.

Topical Quality Assurance Manual, Chapter 5-A, section 6.0 and station procedure S123-I-1.7 (Maintenance Order Preparation, Use and Scheduling) implement requirements for use of maintenance orders during work on safety related equipment. These procedures require that a copy of the applicable maintenance order be present at the job site and followed in a step-by-step manner while the task is being performed.

Contrary to the above, on November 5, 1986, during calibration and functional testing of radiation monitor 2/3 RT-7813, (1) a copy of the applicable maintenance order was not present at the job site; (2) required "as left" valve positions had not been recorded on the valve position verification sheet; and (3) prerequisite 3.14 of procedure S023-II-4.16 had been signed off, although the required verification had not yet been performed.

This is a Severity Level IV Violation (Supplement I) applicable to Units 2 and 3.

Pursuant to the provisions of 10 CFR 2.201, Southern California Edison Company is hereby required to submit to this office within 30 days of the date of this 8701280078 870109 PDR ADOCK 05000206 G PDR
