Pacemaker The Medtronic-Alcatel program operates under safety guidelines developed by a committee of the European Nuclear Energy Agency (ENEA).

Nations represented on the ENEA committee are Auntria, Denmark, Germany, France,

.. Spain, Sweden and Switzerland.

Each hno agreed'to permit entry of such patients on a limited and experimental basis and Medtronic will soon

supply pacemakers to each country for further.. clinical trials.

The ENEA expects other countries to join in the preparation of international standards, j

legislation that severely restricts medical use of Plutonium 238 in pacemakers.

Under present licensing restrictions in the U.S.,

large-scale distribution of the device would be impractical, but' limited clinical L

cvaluation could be done.

The only other nuclear-powered pacemakern in humans were implanted at the Nat,ional' Heart Hospital in London, England on July 15 and July 22, 1970.

The first of these British devices, implanted by Dr. Edgar Sowton, is currently in therapeutic use.

The second was removed after several months because of a slight rate change.

The British project is under

the auspices of Britain's Atomic Energy Authority.

While the Medtronic-Alcatel program has been we.1 received by the European medical community, the number of devices commercially available is still limited.

Medtronic has exclusive world marketing rights to the Alcatel power. source.

Product Data Designed for long, term bipolar stimulation with mycardial leads, the nuclear-powered pacemaker incorporates the same basic circuitry that is used in the Medtronic Model 5943 Unipolar Pulse Generator.

An electronic convertor is added, however, to increase the power source voltage.

The R-wave inhibited demand pulse generator has a preset rate of 72 pulses per minute and a fixed amplitude of five volts.

The pacemaker has an "es. cape rate" feature that allows sinus rhythm to prevail at rates down to 62 bpm.

However, if the ventricular rate falls belou 62 bpm, the. device will pace the faster rate of 72 bpm.

The unit also contains a built-in switch that at is actuated by an external magnet.

In this way, the pacemaker is converted from demand to asynchronous mode which allows testing of the unit during periods when the patient is in normal sinus rhythm (and the pulse generator's e

output is suppressed).

Nuclear-pownred Pacemaker The capsule containing the power source is made of tantalum with an inner core of platinum for gamma radiation shielding.

S,ufficient space is

-' provided between the two layers for helium buildup as the plutonium

,, disintegrates.

The power source and electronics are encased and hermeticall

scaled in titanium, shielding it from most sources of magnetic, i

jolectromagnetic, and radio frequency interference.

The Plutonium 238 power source has a half life of 87 years, theoretically

,.providing sufficient power for at least 10 years of pacing.

The fuel capsule has undergone exhaustive testing to determine that the Plutonium 238 is safely contained.

Some of the destructive conditions considered are those of impact, crush, elevated temperature and pressure.

Manufacture of the pacemaker, including placemant of the power source within the pulse generator, is performed at Medtronic's Minneapolis plant.

e HISTORY OF THE NUCLEAR-POWERED PACEMAKER The first nuclear-powered pacemaker used in a human was implanted in a 58-year-old woman on April 27, 1970.

That historic implant was performed l

i et Broussais Hospital in Paris by a team headed by Drs. Paul Laurens and l

..Armand Piwnica.

To date, more than 50 patients have received the isotopic pacemaker in France, Germany, Belgium and Holland.

Most patients are now leading active lives.

In the U.S.,

the Atomic Energy Commission (AEC) has recently (June 30) granted permission for limited clinical application,of the device at the Veterans' Administration Hospital'in Bu f falo, New York.

The. energy system was developed under a grant from the French Atomic Energy Commission (CEA ), while the pacemaker system was jointly developed by Medtronic, Inc., Drs. Laurens and Piwn'ica, and Alcatel, a French corporation.

The radio-isotopic energy system using Plutonium 238 is manufactured at Alcatel, a company which,has miniaturized the essential thermopile that converts heat to electrical energy.

The plutonium fuel is enclosed in a multi-layer capsule which protects against harmful radiation and container rupture (see illustration).

The human implants in 1970 followed extensive research by the CEA, Alcate'1, Medtronic and the French doctors -- with the entire project under the direction of Dr. Laurens, a cardiologist.

Soon Dr. Piwnica, a heart surgeon, joined the team.

It was through his invitation to Presidsnt Earl Bakken that Medtronic became a part of the program.

After intensive testing, the French Minister of Public Health authorized clinical application of nuclear-powered cardiac pacing.

He acted on the

' safety report of a special committee.

The confectionery store owner who made medical history as the first patiend was suffering from intermittent heart block (when the heart periodically reverts to a slower rate).

The nuclear-powered Medtronic unit was her first pacemaker.

The second patient has been a pacemaker user for many, years.

She is a 49-year-old free-lance photographer living in Paris.

The nuclear-powered pacemaker was implanted in a male patient on March 2,

1971, at Broussais Hospital.

It was his first pacemaker and, like tho,
