Dear Mr. Boulette:
On September 13,1997, the NRC completed an inspection by the resident inspectors and
specialist inspectors from Region I at the Pilgrim Nuclear Power Station in Plymouth,
Massachusetts. The enclosures present the results of that inspection (Enclosures 1 and 2)..
With respect to Enclosure 1, operators averted a reactor trip by responding effectively to a
sudden insurge of seaweed on the intake structure traveling screens. This was an
improved response over a previous event largely due to the implementation of lessons
learned such as developing a control rod rapid power reduction process. The radiation
controls program was found to be effectively implemented with good technical support
program areas. Average BWR exposure performance at your facility continues to rank high
when compared to other BWRs. A review of your cobalt reduction activities have indicated
somewhat limited actions to reduce the cobalt inventory, although we acknowledge the
recent addition of a zinc injection process to attempt to suppress the deposition of cobalt
in the reactor piping systems.
Enclosure 2 is a redacted report of an investigation conducted by the NRC Office of
Investigation (01) to determine if a former receipt inspector was harassed and discriminated
against for raising a concern regarding the Rockwell Hardness Tester. Based on its
investigation, Ol did not substantiate that discrimination occurred. This report is being
provided to you and the individualin light of an ongoing Department of Labor case relative
to this matter, in which you and the individual are parties, based on the investigation, this
inspection as well as inspection Report 97-03 issued July 22,1997, the NRC has found
that certain licensed activities were not performed, as required, yet records were created to
- indicate that the activity was performed. Specifically, in one instance, the individual
signed his name to a Receipt inspection Report indicating that 20 batteries had been
receipt inspected, when in fact, only one of the battery boxes had been opened. Without
the other 19 boxes being opened, the inspection could not have been performed, in the
other instance, the individual signed his name to the Receipt inspection Report indicating
that he took dimensional checks of two swagelock fittings using precisional test
equipment, when in fact, the bags containing the equipment were hermetically sealed and
had not been opened, and an inspection could not have been done without opening the
bags.
