BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - CONDUIT SUPPORT STIFFENER PLATES NOT INSTALLED PER DRAWING - BLRD-50-438/84-34 BLRD-50-438/85-18, BLRD-50-439/85 FINAL REPORT The subject deficiency was initially reported to NRC-OIE Inspector P. E. Fredrickson on May 10, 1984 in accordance with 10 CFR 50.55(e) as NCR 3069. Subsequently, NCR 3457, which documents a similar condition, was initiated and reported with NCR 3069 in TVA's second interim report. Two more items, NCR 3454 and NCR 3684, which document similar conditions, wore initiated and TVA reported on all of these NCRs in the third interim report under CDR No. BLRD-50-438/84-34. A related condition documented in NCR 4254 was reported separately in a first interim report dated June 26, 1985 under CDR No. BLRD-50-438/85-18, BLRD-50-439/85-18. Enclosed is our final report on all of the aforementioned NCRs.

FINAL REPORT Description of Deficiency Numerous deficiencies involving electrical conduit and electrical hanger installations which were initially inspected and accepted during the timeframe from 1979-1984 have been identified at Bellefonte Nuclear Plant (BLN). Several nonconformance reports (NCRs) have been issued by TVA to document the subject ,

deficiencies. NCRs 3684 and 4254 will be used to track these deficiencies and are identified below.

NCR BLN 3684 Due to the conditions identified by previously issued NCRs (NCRs 1069 and 3454),

TVA performed a reinspection of 15 randomly selected electrical hanger installations which had been initially inspected and accepted during the 1979-1984 timeframe. From this reinspection, four (4) supports were identified with various deficiencies such as incorrect bolt sizes, inadequate welds, interferences, incorrect configuration, and incorrect mark number of support installed.

NCR BLN 4254 Numerous " nonsignificant" NCRs (55) have been written to document deficiencies

) in conduit installations in the intake pumping station (IPS) at BLN. These installations were previously inspected and documented (prior to January 1, 1984) as acceptable. This NCR identifies this adverse trend in hanger deficiencies similar to those identified in NCR BLN 3684 above, as well as other conduit installation deficiencies.

TVA has determined that the cause of these deficiencies was a lack of knowledge regarding detailed design and construction requirements by personnel performing electrical conduit support installation and inspection activities during the identified timeframe (1979-1984). After this date, the Quality Manager's Organization was in place with con.plete independence (split) between Engineering and Electrical Quality Cor. trol. Only isolated cases of conditions adverse to quality have occurred since the split.

Safety Tmpilcations i

The failure to construct safety-related electrical conduit and conduit supports to design requirements could possibly result in a failure of the affected systems during design basis loading conditions. This could adversely affect the 4

safe operation of the plant.

Corrective Action TVA will' reinspect and rework, as necessary, all conduit at Bellefonte that was inspected and documented before January 1, 1984. TVA's Division of Nuclear Engineering (DNE) has established the reinspection and rework criteria for

! Division of Nuclear Construction (NU COM) to use in the raceway reinspection. A special status code, "0S", has been added to the test column of each conduit i

inspection document with an initial inspection completed prior to January 1, 1984, as listed in the Engineering and Construction Monitoring and Documentation System (ECM&D) computer program. All conduit inspection documents with the "0S" code will have all related reinspections performed. Any discrepancies identified during the reinspection will be evaluated by TVA and resolved on a

case-by-case basis in accordance with the applicable site procedures.
Electrical hangers will be reinspected and reworked in accordance with the requirements of BNP-QCP-3.32, " Raceway Verification."

The actions to prevent recurrence has already been implemented as a result of the split between Electrical Quality Control and Engineering and by

implementation of training programs wherein personnel were trained to all procedures which affect their work and have been trafaed to each subsequent 1

! procedure revision. As a result of these actions, subsequent construction '

inspections show only isolated cases of conditions adverse to quality due to )

inspection error having occurred.

TVA will be in full compliance for units 1 and 2 six months prior to their l respective fuel load dates.
