  • Attachment I Violation:

10 CFR 20.201(b) requires that each licensee make such surveys as may be necessary to comply with all sections of Part 20. As defined in 10 CFR 20.201(a), " survey" means an evaluation of the radiation hazards incident to the production, use, release, disposal, or presence of radioactive materials or other sources of radiation under a specific set of conditions.

Contrary to the above, during the pump seal replacement on recirculation pump "D" on the 23' elevation in the drywell, no surveys were made to assure compliance with 10 CFR 20.202, which requires appropriate personnel monitoring. Specifically, between July 30 and August 7,1986, inadequate surveys were made, in that radiological measurements were not representative of the work area radiological conditions. As a result, appropriate personnel monitoring was not provided and an unplanned radiation exposure to one of the workers in excess of your administrative limits did occur.

Response

GPUN concurs with the violation.

1. The corrective steps which have been taken and the results achieved:

(1) The work on the pump seal was stopped and detailed surveys of the work area were performed.

(2) The dosimetry placement required by the RWP was changed to reflect the maximum whole body dose received.

(3) The incident was reviewed by all Radiological Control Technicians (RCTs), Group Radiological Control Supervisors (GRCSs) and Radiological Engineers. l (4) A copy of the Radiological Incident Report was placed in the ALARA file for review prior to performing future seal replacements.

2. Corrective steps that will be taken to avoid further violations:

(1) A discussion of the incident will be incorporated in cyclic training for RCTs and GRCSs.

(2) We are evaluating specific criteria for survey frequency and detail based on the type of work to be performed.

3. Date when full compliance will be achieved:

Full compliance was achieved on October 20, 1986.
