Company personnel determined that temporary unescorted access to the PVNGS Protected and Vital Areas was granted to a contract employee whom it was later discovered had falsified information on his Personal History Statement by failing to disclose that he had been denied access at another nuclear power facility.

Based on this information, Access Authorization personnel have determined the individual would not have been granted unescorted access had the information been disclosed.

The individual accessed vital areas in Unit 2 only, while supporting refueling activities and there is no evidence to suggest the individual committed or attempted: theft or diversion of special nuclear material, physical damage to a power reactor, or interruption of normal operation of PVNGS.

The PVNGS Security, Radiation Protection and Operations Departments were alerted to the condition and potentially affected radiological surveys were re-performed.

Potentially affected systems have been inspected during normal operator rounds and these systems are currently being tested in accordance with plant start-up procedures.

The individual's personal history information has been entered into the Personnel Access Data System (PADS) to alert other nuclear licensees.

Introduction:==

l On September 19, 1997, at approximately 1100 MST, Palo Verde Units 1 and 3 were in MODE 1 (POWER OPERATION) operating at approximately 100 percent power and Unit 2 was in MODE 6 (REFUELING) with the Reactor Core (AC) off-i loaded to the spent fuel pool (ND), when Arizona Public Service (APS) personnel determined that temporary unescorted access to the PVNGS Protected and Vital Areas was granted to a contract employee whom it was later discovered had falsified information on his personal history i

statement by failing to disclose that he had been denied access at another nuclear power facility.

Besed on this information, Access Authorization personnel have determined the individual would not have been granted unescorted access had the information been disclosed.

On August 27, 1997, APS granted temporary unescorted access privileges to a contract Radiation Protection (RP) technician, in accordance with APS l

procedures, while the individual's background information was being i

verified.

i On September 15, 1997, at approximately 1830 MST, the contract employee self-terminated employment with his contract organization and as a result his access was suspended.

During the period of time between September 15 and September 19, 1997, APS was made aware that the contract employee may have been denied access at another Nuclear "ower Facility.

APS Access Authorization personnel initiated suitable inquiries to substantiate the information. Also, the observed behaviors of the contract employee prompted APS management to evaluate the employee's performance against Fitness for Duty Standards.

The Access Denial occurred in 1991 and was therefore not included in the Personnel Access Data System (PADS) Database

and was beyond the five year interval required to be verified by regulatory requirements. Access Authorization personnel performed a review of the contract employee's Personal History Statement and determined the individual had falsified the statement by omitting the j

previous access denial, j

At 1149 MST, September 19, 1997, APS reported this event to the NRC via d

the Emergency Notification System as a 1-Hour Non-Emergency event (ENS 32961) in accordance with 10CFR73.71.

No safety systems were affected or threatened during this event. The individual accessed vital areas in Unit 2 only, while supporting refueling activities. At this time there is no evidence to suggest the individual committed, or attempted, or made a credible threat to commit or cause significant physical damage to the power reactor or its equipment, nuclear fuel or spent nuclear fuel, or interruption of normal operation of PVNGS.
