Dear Mr. Peterson:

On August 30, 1997, the NRC completed an inspection at your Catawba Nuclear
Station. The enclosed report presents the results of that insr .ction,
During the six-week period covered by this ins
occurred when Digital Optical Isolators (D0ls)pection, two manual tripsin the seal-in

associated with the D Steam Generator (SG) Main Steam Isolation Valve (MSIV)
failed, causing the MSIV to close.
The DOI failures are of concern because of
their previous reliability record and because of their )otential generic
impact to important control functions in applications tlat are safety-related
and critical to plant operation.

Two violations of NRC requirements were identified and cited in the enclosed
. Notice of Violation (Notice),
Described in detail in the enclosed re) ort, the
first violation concerned maintenance activities following the first

failure that were not conducted in accordance with electrical connection
diagrams and the controlling procedure: thereby, adversely affecting plant
cooldown and requiring manual action by control room operators to mitigate the
impact. The second violation. which involved missed P-11 and'P-13
l
surveillances, has raised concern over the completeness and thoroughness of
your review associated with Generic Letter (GL) 96-01. Specifically, this
violation may be indicative of other shortcomings in your GL 96-01 review.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response. The NRC will
use your response, in part, to determine whether further enforcement action is
n=essary to ensure compliance with regulatory requirements.
