A listing is needed for all physicians involved in CCF's pacemaker program.
The listing should indicate:

Physician's name 2.
Office address 3.
Telephone number 4.
State in which licensed to practice medicine 5.
Specialty Brard Certification 6.
Position with CCF 7.
Description of physician's previous experience with pacemakers (not necessarily nuclear pacemakers) 8.
A statement signed by participant physicians certifying that they are familiar with the manufacturer's NRC approved protocol and that they agree to follow that protocol.
Kindly foward this information to me as soon as possible so that we can renew our Special Nuclear Materials license.
The license is dated to expire June 30, 1986.
