Dear Mrs. Vacca-Enclosed is a copy of the mcnbcrs of the team including two thoracic surgeons - Doctor Groves and Doctor Irarrazaval and Doctor Morant of the Cardiology Department who will be participating in our program.
All physicians are aware of the protocol and this will be under direct supervision by myself and the protocol will be adhered to by the Cleveland Clinic.
Enclosed is a copy of the instructions for hospital or clinic operators notified here at the Cleveland Clinic.
'Ihe patients are followed on a regular basis by telephone transmission system on a two month basis. 'Ihey also cxxne to the Cleveland Clinic on a 3-4 month basis for check-ups where they are actually seen by one of us physicians in the Out Patient Department. At that time we could determine if the patient is carrying the I.D. card ard wearing the approved jewelry.
Enclosed is a copy of an application frcm April of 1976 signed by Mr.
Gilbert Cook, Hospital Administrator and Doctor Sheldon, Head of the Department of Cardiology and also a mcmber of the Board of Governors here at the Cleveland Clinic. I think this would suffice as to a statcment of the Clinic's long-term comtitment and responsibility for paccmaker follow-up in the cover of nuclear paccmakers.
