Dear Sir or Madam:
This is to request a renewal of license for our U-238 pacemakers.
In response to the questions posed for renewal, please note:
1.
License - SNM-1412 Expiration Date - 9/30/83 Program Code - 22160 2.
Abington Memorial Hospital 1200 Old York Road Abington, Pennsylvania 19001 3.
Principal reason for requesting renewal of licence is to continue to follow ten patients who have had plutonium pacemakers implanted.
These patients are checked by telephone directly or through the referring physician at least once every six months. A copy of the latest status of these patients is enclosed.
In the past, two pacemakers have been returned following death of patients.
4.
No new pacemakers will be implanted, and there are none physically in the institution.
5.
All pacemakers have been under the supervision of Stephen B.
Knowlton, Jr., M.D.
6.
There have been no changes in the commitments contained in Condition 16 of the license.
