In accordance with Nine Mlle Point Unit 1 Technical Specification 3.6.10.1 and 6.9.2.b, we hereby submit the following Special Report concerning inoperable penetrations.

On September 4,1986, the following Fire Barrier Penetrations were declared nonfunctional: 4RF-21, 24, 26, 29, 32, 37, 40, 44, 48, 61, 62, and 77.

These penetrations were disturbed during the 1986 refueling outage for the replacement of four containment spray heat exchangers.

These penetrations were not breached. However, associated piping work damaged the penetration 4

seals.

In accordance with Technical Specification 3.6.10.1.c, the fire detectors on one side of the nonfunctional fire barrier were verified, and a fire watch patrol was established.

The penetrations have been subsequently repaired and were returned to an operable status on October 3, 1986.

On September 3, 1986, Fire Door D-107 was found to have a closure problem, the door was rubbing against the side jamb. On September 29, 1986, work to repair the closure problem was completed.

However, at that time a nonconformance report was written against Fire Door D-107 since it would not close by its own power. The positive Auxiliary Control Room pressure was greater than the door's closure power. On October 14, 1986, the door closure was reworked to eliminate the door closing problem.

This rework failed to correct the problem.
