Dear Mr. Keppler:
The above references provided information concerning a deficiency reportable pursuant to 10CPR50.55(e) concerning a large laminar indication in the steam generator snubber serial number 4 piston rod end blade supplied by the Boeing Engineering and Construction Company for Byron and Braidwood Stations.
The purpose of this letter is to provide an updated status of the corrective actions taken to resolve this issue and is considered to be a Final Report.
We have completed the examination of the Steam Generator Snubber materials.
The examination was accomplished by ultrasonic testing using ITT Grinnell's approved procedures (UTE-1 Rev. 5).
The acceptance criteria used was the ASME Code Section III 1974 Winter 74 addendum.
The results of the exam yielded 10 end caps and 1 rod end with unacceptable indications.
The rod end and 6 end caps were replaced.
2 end caps were weld repaired to within code requirements and an analysis was performed which accepted the last 2 end caps.
