SUBJECT: NRC AUGMENTED INSPECTION TEAM REPORT NOS. 50-280/86-42 AND 50-281/86-42

This refers to the Nuclear Regulatory Commission (NRC) augmented inspection

conducted by Mr. V.'W. Panciera during the period of December 9, 1986 through

January 14, 1987. The inspection included a review of activities authorized for

your Surry facility. At the conclusion of the inspection, the findings were

discussed with those members of your staff identified in the enclosed inspection

report.

Areas examined during the inspection are identified in the report. Within these

areas, the inspection consisted of selective examination of procedures and

representative records, interviews with personnel, and observation of activities

in progress.

Your letter of January 14, 1987, which forwarded your report entitled "Surry

Unit 2, Reactor Trip and Feedwater Pipe Failure," provided detailed information .

on the December 9,1987 event and a recovery plan and corrective actions for NRC a

review and concurrence prior to station restart. We have reviewed your report

and concur with the actions you plan to take prior to station restart.

The inspection findings indicate that certain activities appeared to violate

NRC requirements. The violation, references to pertinent requirements and

elements to be included in your response are described in the enclosed Notice

of Violation. In your response to the violation, you may reference certain

paragraphs of the above referenced report when discussing corrective steps that

have been taken.

I During the week of January 12, 1987, a subsequent NRC inspection was conducted

to evaluate the spurious fire protection system actuations which resulted from

this event. Our assessment of your fire protection system spurious actuation

evaluation and your corrective actions associated with preventing recurrence

l 1s discussed in Section 13.b of this report. It should be noted as a result

i of our assessment we are concerned with personnel safety and control room

habitability due to inadvertent or actual operation of the carbon dioxide

,

fire suppression system. Therefore, the fire protection program and system

enhancements outlined in our inspection report and discussed with your staff

i should be considered for implementation.

During this inspection, the Surry secondary chemistry program was reviewed as a

possible contributor to the pipe rupture. At the request of the inspector, the

plant staff agreed to review the procedures related to oxygen and pH control.

We believe that this review should be completed expeditiously to ensure that

chemistry control maximizes the integrity of carbon steel as well as that of

stainless steel and alloy-600 components.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this letter and its enclosure

will be placed in the NRC Public Document Room.

The responses directed by this letter and its enclosures are not subject to the

clearance procedures of the Office of Management and Budget as required by the

Paperwork Reduction Act of 1980, PL 96-511.

We appreciate your cooperation during our team inspection and subsequent

evaluation of this event. Your voluntary quarantine of components and systems

was very beneficial as was your open discussion of your findings and evaluations.
