INITIAL STARTUP ADMINISTRATIVE MANUAL ISA-006 I INITIAL STARTUP TEST PROCEDURE CHANGES REVISION NO. 3 PAGE 2 0F 14 1.0 Purpose This procedure prescribes the administrative methods of controlling the preparation, review, approval, distribution, and implementation of permanent Initial Startup Test Procedure changes which become necessary during conduct of testing. These changes shall consist of permanent changes incorporated directly into the existing revision of the ISU procedure.

2.0 Applicability This procedure applies only to Initial Startup Test Procedures

' contained in the Initial Startup Test Manual. Ihis procedure shall apply after conduct of testing has begun in a procedure or after the final revision of the procedure has been issued as determined by the ISU Coordinator.

3.0 Definitions 3.1 ISU Procedure Change (IPC) - A permanent change incorporated into an ISU procedure without the necessity of a procedure revision.

( Intent Change - A change to any of the following, as described by the FSAR; test objective, acceptance criteria, or test method. ~

4.0 Instructions l

4.1 General i

l 4.1.1 Changes shall be classified as either intent change or non-intent change. -

4.1.2 Changes may be originated by any qualified ISU Test Engineer. The Originator shall obtain an " Initial Startup Procedure Change Form" (IPCF, Form ISA-006-1, Attachment

1) and complete it in accordance with Section 4.2.

4.1.3 The ISU Coordinator shall maintain an " Initial Startup Procedure Change Log" (Form ISA-006-3, Attachment 3).

Each IPC shall be assigned a sequential number using the following format:

YY - XXX -

YY is the last two digits of the current year and XXX is a sequential number assigned to each IPC for that year.

Example: The tenth IPC issued in 1985 would be 85-010.

Shif t Test Director, and Shift Supervisor prior to performance of the change. Intent changes shall also be approved by the SORC prior to implementation.

4.2 Origination 4.2.1 Enter the ISU procedure number, procedure title, and originator in the IPC log with the next available sequential number.

4.2.2 Enter the originator, IPC number, ISU procedure number, revision number, and procedure title on the IPCF.

4.2.3 Determine whether or not the procedure change should be considered an intent change or a non-intent change, and check the appropriate box on the IPCF to so indicate.

4.2.4 Complete the " Change" section of the IPCF. This section should refer to an attached, marked-up copy of the affected pages. Continuation page(s) ("IPCF Continuation", Form ISA-006-2, Attachment 2), with appropriate heading information completed, may he used as required. -

4.2.4.1 For changes which will require the affected procedure pages to be " word processed", the l

" change" section of the IPCF shall identify all

existing IPC's on the affected pages which are .;

incorporated by the new change. (e.g. Under ~

the " change" section state "This change -

incorporates IPC-005 and IPC-009 on pages 45, 47, and 48.")

4.2.4.2 Obtain a copy of the affected procedure pages from Control Copy 63 or 11A and mark it up as required to show the procedure change (s). l 4.2.4.2.1 For changes which are to be implemented directly into the existing pages of the procedure, enter the IPC number next to each change made in the procedure.

4.2.4.2.2 Any new pages which are added should be given sequential and unique page

' designations (i.e. pages added between 10 and 11 should be labeled 10.1, 10.2, 10.3, etc.). Enter the IPC number in the page number block of each added page.

(retyped pages) shall be identified by entering the IPC number in the page number block. The lines most recently changed by the IPC should be identified by a vertical line in the right margin.

4.2.4.3 Attach the marked up procedure pages and, if applicable, the word processed or typed copy of the pages, to the IPCF.

4.2.5 Complete the " Justification" section of the IPCF by giving

a brief explanation of the reason for the change.

Continuation page(s) ("IPCF Continuation", Form ISA-006-2, Attachment 2), with appropriate heading information completed, may be used as required.

4.2.6 The original IPCF and the revised pages shall be reviewed 4

by the responsible test engineer, the shift test director, and the shift supervisor. Concurrence with the change shall be indicated by signatures and dates in the designated locations on the IPCF. ..

4.2.7 Complete the " Required SORC Approval Date" information on the IPCF:

4.2.7.1 If the change is an intent change, mark the

" Prior to implementation" block. SE i

4.2.7.2 If the change is a non-intent change, mark the i

"14-day" block and enter the date corresponding to 14 calendar days after the change is to be implemented.

NOTE: For intent changes only one copy of the IPCF and the attached revised pages will be required prior to SORC approval.

4.2.8 Make copies of the IPCF and the attached revised pages as i

required for the procedure Test Copy, Control Copy 63, and Control Copy 11A.

l 4.2.9 Fill in the IPC Number, Date Implemented. Entered By, and Description columns on the " Initial Startup Procedure Change Index" (Form ISA-006-4, Attachment 4), located at the front of the Procedure Changes section of the affected

4.2.11 Forward the original and one copy of both the IPCF and the attached marked up pages to the ISU Coordinator.

NOTE: For non-intent changes implementation of the change may proceed in accordance with section 4.3 prior to conduct of.the following three steps.

4.2.12 For non-intent changes:

  • 4.2.12.1 Remove the affected pages from Control Copy 63 and maintain them in a separate file along with the copy of the IPCF. Insert the copy of the revised pages in place of the affected pages in Control Copy 63.

4.2.12.2 Remove the affected pages from Control Copy 11A and discard. Insert the copies of the revised pages in place of the affected pages in Control Copy 11A. ._

l NOTE: For intent changes the following two steps and subsequent SORC approval of the change must be completed prior to implementation of the change.

4 4.2.13 A safety evaluation shall be performed in accordance with '

STA-707, " Safety Evaluations".

NOTE: For immediate SORC approval, the Office Services Group should be verbally notified of the urgency when the following step is performed. The Office Services Group should also be notified at this time of any previous verbal agreements made with SORC on a meeting time.

4.2.14 Forward the completed STA-707 form, the original IPCF, the marked up page(s) and, if applicable, the re-word processed or re-typed page(s) to the Office Services Group for submittal to the SORC as specified by section 4.4.

NOTE: If testing is to continue with a retest after implementation of the IPC, do not enter the IPC in any part of the procedure Test Copy that will be performed in the retest section.

4.3.1 Remove and discard all affected pages that do not contain signatures from the procedure Test Copy and/or insert the revised pages. Stamp each inserted page " Test Copy".

4.3.2 If the affected pages do contain signatures the IPC shall be entered by one of the following methods:

4.3.2.1 If text is to be deleted or if a limited amount of text is to be added or revised, the change may be added by typing or using a black ballpoint pen, as described in the " Changes" section of the IPCF. Enter the IPC number next to each change made in the procedure Test Copy. ~

4.3.2.2 Insert the retyped or revised page in the procedure Test Copy. Stamp each inserted page

" Test Copy" and:

4.3.2.2.1 If added pages include steps which are shown on previous page(s) but are not -

yet signed as completed, anter "If/A" in the signature block of the original pages and initial and date. Annotate the original steps to indicate that the step (s) will be signed on another page (e.g., "This step restated and performed on page 22.1.").

l 4.3.2.2.2 If added pages include steps which are shown on previous page(s) as being completed and signed off, enter "N/A" in the signature block of the added page and initial anti date. Annotate the steps to indicate that the steps

after receiving approval from the ISU Coordinator.

1 4.3.2.3 If added page(s) are replacing page(s) which have completed and signed steps, the ISU Coordinator or his designee (s) shall transfer the signatures and dates from the original pages to the added pages with an annotation to indicate this. (e.g., " Sign-off for Test Engineer per IPC 84-020 by ISU Coordinator").

The original pages shall be placed in the Attachments section of the procedure Test Copy.

4.4 SORC Review and Approval 4.4.1 Prior to Implementation (Intent Change)

Certain IPC's involving intent changes may become critical path to the project schedule and will require expeditious review and approval per the following steps:

4.4.1.1 The ISU Coordinator shall arrange for the earliest possible review of the IPC by the SORC.

4.4.1.2 The SORC review, as a minimum, shall evaluate the following: _

4 4.4.1.2.1 Whether or not the IPC changed the '

intent of the procedure.

4.4.1.2.2 Whether or not the IPC represents an unreviewed safety question.

4.4.1.3 Upon resolution of SORC questions and concerns, the IPC shall be approved by the SORC Chairman, or his designee, by signing and dating the IPCF.

4.4.1.4 The approved IPCF and the Safety Evaluation Form shall be handled in accordance with section 4.5,

" Retention".

4.4.2 14-Day Approval (Non-Intent Change) 4.4.2.1 The Office Services Group shall ensure that a SORC review is scheduled not later than 14 calendar days after implementation of the IPC.

INITIAL STARTUP ADMINISTRATIVE MANUAL ISA-006 INITIAL STARTUP TEST PROCEDURE CHANGES REVISION NO. 3 PAGE 8 0F 14 4.4.2.2 The IPCF and attachments shall be presented to the SORC by the Office Services Group.

4.4.2.3 The SORC shall review the IPC not later than 14 days after implementation of the IPC. The review, as a minimum, shall evaluate the following:

4.4.2.3.1 Whether or not the IPC changed the intent of the original procedure.

4.4.2.3.2 Whether or not the IPC represents an unreviewed safety question.

4.4.2.4 If it is determined that an IPC modifies the intent of the original procedure, involves an unreviewed safety question, and/or causes or could cause a violation of Technical or Environmental Specifications, the following shall be performed:

4.4.2.4.1 Conduct of the procedure shall cease immediately if still in progress.

4.4.2.4.2 A problem report shall be issued in accordance with STA-504 " Problem Report".

4.4.2.4.3 Make an appropriate entry in the 3 procedure test log.

l 4.4.2.4.4 Enter " disapproved" in the "SORC Approval Date" block on the " Initial Startup Procedure Change Index" form.

4.4.2.5 When the change is approved, the SORC Chairman, or his designee, shall sign and date the IPCF.

4.4.2.6 The approved IPC shall be handled in accordance with section 4.5, " Retention".

4.5 Retention 4.5.1 One copy of the SORC approved IPCF should be made and forwarded to the ISU Coordinator.

4.5.2 The approved IPCF, approved Safety Evaluation form, marked up pages and, if applicable, the re-word processed or re-typed page(s) shall be forwarded to the Office Services staff for transmittal to the TUCCO Records Center per STA-302.

4.$.3.1 Non-Intent Changes 4.5.3.1.1 Insert a copy of the approved IPCF in place of the non-approved copy in the Procedure Changes section of the procedure Test Copy. Discard the copy of the non-approved IPCF.

4.5.3.1.2 Enter the IPC SORC approval date en the Procedure Change Index, Form ISA-006-4, of the affected procedure.

4.5.3.1.3 Discard the original affected pages removed from Control Copy 63 and the copy of the IPCF, which are being maintained in a separate file.

4.5.3.2 Intent Changes 4.5.3.2.1 Insert a copy of the approved IPCF in place of the non-approved copy in the _

Procedure Changes section of the Procedure Test Copy. Discard the copy of the non-approved IPCF.

4.5.3.2.2 Enter the IPC SORC approval date on },

the Procedure Change Index, Fore ISA-006-4, of the affected procedure. ~

4.5.3.2.3 Remove the affected pages from Control Copy 63 and 11A and replace them with a copy of the revised pages. Discard the original pages removed from Control Copy 63 and 11A.

l 4.5.3.2.4 Proceed with implementation of the change in accordance with section 4.3.
