I have enjoyed boing a part of Brown and Root en this project.

i I found large air pockets or voids and in many cases found powdery concrete at times so powdery that it would not even form a core. This was found all i

over the plant includir:g the reactor area and in both reactor containments, This was reported to the proper Grown A Root peopic but nothing was ever done to correct it as far as 1 know.

Also during this time, there were other things going on that Caused a lot of discomfort for me and c.y f amily. I was accused of stealing something j

that never lef t the jobsite. I was asked to take a polygraph test and told that if I took it my job was in Jcopardy and if I did take it W ob was i

in jeopardy. At this time I was making something for was going to be used at his house. Also, things were made for for his personal use at his hace using material which was bought for use at l

stated but none could provide anything more sub ive then their personnel opinions..The SRIC has attempted to detemine the probable effect of having an indeterminate amount of clay or other possibly deleterious substances o

mixed with an unkncwn amount of concrete. W dccording to the project

l specifications, the aggregates may contain up to 8 percent by weight of clay and~still be acceptable. Since the specific gravity of damp clay and sand and gravels (used for aggregate) are similiar, the SRIC calculated the allcwable amount of clay that could be in a cubic yar'd of concrete.

This p.:mber came to approximateyf@ pounds and therefore in a typical ten cubic yard load, a total of 1200 pounds could be in the concrete and still

/4 d e;%d allowable. The SRIC reviewed the acceptance test record t' hat weEe Yequired to be conducted monthly and found that only rarely did the amount of clay in the stock h="

percent, leaving a substantial margin exceed 1 percent and was generally und a

The Bureau of Reclamation " Concrete Manual" indicates that the presence of large amounts of clay and like contaminates in the aggregate may increase the mixing water requirements of tne concrete.';' may make the cencrete more susceptible weathering,* or may be weak and may hinder the hydration of the concrete. Althoegh there is no absolute certainity that the particular loads of concrete contaminated were tested since not every load is or was, the cumulative data has not shown any significant increase in the mix wate-require loss of strength in the standard compression tests. [/ The SRIC examined the present layout of the batch plant to identify the routing of <

scale control cables. At present only thd small dry batch plant remains installed w the larger central mixer plant having been removed. The SRIC can state based upon; personal observations and.on interviews with plant personnel who were working there -

during at least the past five years that the layout of the two plants were similiar and that the remaining plant layout is essentially unchanged from what it was from b:snnn'-

the 4e+ng cfi the project. The SRIC found that the cable from aggregate scale,C/

would c be easily manipulated by a person standing in the doorway of the scale building.

The cable from the water and cement scales could also b,e manipulated but with considerable difficulty since they enter the scale,flC/L.1,~..*Ybuilding at roof heigtit from the much higher reaches of the plant. Also anyone entering the scale't' Ivc!!!abr building could have sive nth manipulated any or all of the cables. The effect of such manipulation woul(,cause' the scales in each case to read as if there was more material in the scale hopper than there actually was. This would in turn cause the semi-automatic control. system

alleger stated that the scale cable manipulation ususally was of the scale weighing l

ice inte the batch if the supply of ice was running short. The SRIC has reviewed the l

effects of under-weighing each of the ingredients involved:

The calibratIand automation of the batch plant are relied upon to produce concrete '

as expedient as possible. The testing of the concrete is relied upon to assure that operating parameters of the plant are stable and that the quality of the ingredients are eis adequately stable. It thus appears that th'e allegation had factual merit in that the scale cables could have been manipulated and perhaps were on occasions, The testing program however prov' ides' reasonable assurance that any concrete producet under the destablizing influence df such manipulation would have been detected or ha-

.had such little effect as to be irrelevant to the strength of the in-place concente.

c. The SRIC made no specific attempt to locate the record that would identify the alleged missing hold point for a weld on one of the joints in one of the pools since there are thousands of such records and the time frame was not identified nor was the pool identifed in which the incident occurred.

The SRIC did however review several records pertaining to the south pool in the fuel building. These records were all very similiar in that there were two hold points established for each joint. These were a hold point for initial fitup and cleanless prior to weldin and a final Soldpoint for visual inspection and testing of the joint after welding.

Since the testing was by liquid penetrant and was perfomed by both the craft and QC, this hold point would not be likely to have been missed. Missing the first hold point on occasion undoubtably happened on occasion since the craft must notify QC that they are ready for the inspection and for various reasons this notification was not provided. Both the fitup and, cleanliness of the joint would have a bearing e

on the quality of the finished weld if substantially improper. Should the visual

appearance' of the weld and the liquid penetrant test both be satisfactory, the SRIC believes that the weld would be satisfactory for its intended purpcse which is to seal the joint to leakage of the water. in the pool. The SRIC accepts that the allegaticn is true t.ut also finds that there is no safety significance in the matter

d. Based on the infomation provided by the alleger, the SRIC was able to locate NCR dated i hich containes the allegers signature. The alleger had signed'in the fcm space 1abeled6"co1struction review / approval" below the space where the disposition instructions are provided for the noted noncenfomance. The other spaces which require signature approval of the disposition are those of an engineer and quality assurance. The purpose:df the signatures is to denote that all three parties agree with the stated disposition. Construction review of the dispos-ition essentially means that the construction forces agree that they can do what instructions state. The other signatures essentially mean that engineering agreet

that the disposition is sound from an engineering standpoint while the QA signature indicates that when the instructions are accomplished as stated, the product or component involved will be acceptable in relation to the project specifications and drawings. Again 'the allegation is factual but has no safety significance nor does it' wL violate site procederes do not digitate by name or title who must sign the forms, only that all required signatures be present and representative of the groups at the time.

We have been orally informed by the Division of Project Management, NRR that fuel pool liners am not required to be designed and erected to Seismic Category I requirements.

The basis for this position is that the primary function of the pool liner is to provide a leak tight barrier

and a surface suitable for decontamination, rather than to serve as a critical safety structure.

Furthennore the probability of large leaks occurring and being undetected over a period of time such that a potential hazard might be incurmd is acceptably low.

We wish to emphasize however, that this decision does not effect the classification of the fuel pool storage structure, storage racks, supports and piping, all of which are required to be designed and erected to Seismic Category I requirements.
