SUBJECT:

NRC INTEGRATED INSPECTION REPORT 50 354/97 04 AND NOTICE OF

VIOLATION

Dear Mr. Elinson:

On July 12,1997, the NRC completed an Inspection at your Hope Creek reactor facility.

The enclosed report presents the results of that inspection.

During the period, both visiting and resident inspectors conducted numerous routine

inspections of your site, in general, the results of these inspections indicated generally

good performance in the control and monitoring of safety-related activities. A focused

solid radwaste/ transportation program inspection concluded that performance in this area

was strong, although one violation of shipping paper requirements was identified. This

violatioi, which is applicable to both your Salem and Hope Creek units, is referenced in

NRC Inspection Report 50 272/9712 and 50 311/0712 for the Salem facility.

Additionally, several plant power changes were conducted during the reporting period with

,

appropriate vigliance and adherence to established safety practices. Quality Assurance

department audits, supervisory oversight reviews and departmental self assessments were

effective in identifying deficient conditions at the facility and enhanced the overall safe and

reliable operation of the plant.

We did note, however, continued occurrences of human performance errors which either

placed the facility in violation of regulatory requirements or caused significant challenges to

plant management and personnel. We are particularly concerned with your apps atly non-

conservative June 17,1997 decision to remove low pressure emergency core

~ ng

subsystems (ECCS) from service without fully understanding the impact of a concurrent

degraded condition in the high pressurc .olant injection system (HPCI). While the noted

condition would not have prevented initial HPCI system operation upon receipt of an

initiation signal, this decision led to the simultaneous inoperability of two trains of ECCS, a

condition not permitted by Hope Creek technical specifications for operational condition 1,

and is the subject of the attached Notice of Violation.
