Inoperable High Pressure Water Fire Protection System

Dear Dr. Murley:

Technical Specification 3.13A.3.b. (ii) requires a Special Report within fourteen (14) days if the high pressure water fire protection system i beccmes inoperable. On March 28, 1986, the diesel fire pump and two electric fire pumps were out of service, rendering the high pressure water fire protection system inoperable.

On March 11, 1986 electric fire pump No. 11 and two associated pressure maintenance pumps were taken out of service in conjunction with. scheduled work to replace pipe sections of the 10" fire protection loop located in the Unit No. 1 Turbine Building. (See Con Edison, M. Selman to T. Murley, April 7, 1986.) Electric fire pump No. 12 was used to maintain pressure in the remainder of the fire water system. On March 27, 1986 a leak developed in No. 12 pump seal which required taking the pump out of service for repair. The diesel fire pump was then used . to maintain the fire water system pressure.

On March 28, 1986, at 1:45 p.m., the diesel fire pump tripped and could not be restarted. Work was initiated immediately to determine the cause of 'the trip. Concurrently, the local fire department was notified, all' hot work (cutting, welding, burning and open flames) was suspended and three (3) roving fire watches were established. At approximately 6:15 p.m. a loose wire on the diesel fire pump engine governor was discovered and reconnect-ed. The diesel successfully started and a post maintenance test was initiated. The . diesel fire pump and the No. 12 pump were returned to service at 8:00 p.m. (See Con Edison, M. Selman to T. Murley, March 31, 1986.)

To support additional maintenance on No. 12 pump, which began April 2, and reduce service requirements for the diesel fire pump a temporary bypssa connection between the high pressure water fire protection system and the j l pressure maintenance pumps was installed. This permitted the maintenance '

pumps to be utilized to maintain pressure in the system. No. 12 pump will-return to service April 12.
