NIAGARA M0 HAWK POWER CORPORATION NINE MILE POINT - UNIT 2 DOCKET NO. 50-410 Final Report For a Problem Concerning Rejectable ASME III Class 1 Fittings (55(e)-86-02)
Description of the Problem It was determined by Field Quality Control personnel that 22 fittings out of 124 ASME III Class 1 fittings supplied by Alloy Stainless Products Company, exhibited rejectable indications as found by liouid penetrant examination.
The 22 fittings in ouestion did not meet the reouirements of ASME III NB-2546.3 as reoutred by the procurement specification.
The 22 fittings were identified and re.iected prior to use.
The 124 fittings represent all material on site that we have identied as being supplied by Alloy Stainless Products Company.
Analysis of Safety Implications Further investigation of the size and nature of the indications would be necessary to determine the probability of failure of the fittings if installed.
- However, the fittings have been scrapped and are not available for evaluation.
Therefore, had the defects gone uncorrected, the safety of operation of the plant may have been adversely affected.
Corrective Action The 22 defective fittings have been scrapped.
