Gentlemen:

This refers to the inspection conducted by Messrs. J. G. Luehman, B. A. Breslau,

and T. R. Staker of this office during the period December 1-31, 1986, of

activities authorized by NRC Operating License NPF-38 for Waterford Steam

Electric Station Unit 3, and to the discussion of our findings with N. S. Carns

and other members of your staff at the conclusion of the inspection.

Areas examined during the inspection included plant status; Licensee Event

Report (LER) followup; monthly surveillance; monthly maintenance; routine

operational safety inspection; followup on previously identified items;

potential generic problems; radiation protection; preparations for refueling;

and refueling operations.

Within these areas, the inspection consisted of

selective examination of procedures and representative records, interviews with

personnel, and observations by the NRC inspectors. The inspection findings are

documented in the enclosed inspection report.

During this inspection, it was found that certain of your activities were in

violation of NRC requirements. Consequently, you are required to respond to

this violation, in writing, in accordance with the provisions of Section 2.201

of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations.

Your response should be based on the specifics contained in the Notice of

Violation enclosed with this letter.

Also during this inspection, it was found that certain of your activities

appeared to deviate from a commitment made to the NRC. This item and reference

to the commitment is identified in the enclosed Notice of Deviation. The

circumstances surrounding the failure to revise an operating procedure within

the time period specified in the Licensee Event Report have been reviewed;

therefore, no written response to the Notice of Deviation is required.

Management attention should be directed toward ensuring that all commitments to

the NRC, especially those involving a significant event such as a loss of

shutdown cooling, are completed in a timely manner.

The response directed by this letter and the accompanying Notice is not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, PL 96-511.
