STAFF ACTIONS RESULTING FROM THE INVESTIGATION OF THE JUNE 9,1985 DAVIS-BESSEEVENT(NUREG-1154)
Mr. Dircks'. memo of August 5,1985 requested a six-month status report on assigned responsibilities for generic and plant-specific actions resulting from the Davis-Besse event investigation. The five actions assigned to IE have been ccmpleted, as described below:

Reliability of the AFW containment isolation valves and other safety-related valves.
Determine if further generic correspondence, such as an NRC Bulletin, (j) is warranted on this type /cause of failure cf safety-related valves.
IE Action:
In response to this item, it was determined that improper setting of torque and limit switches on motor-operated valves was a significant contributor to the Davis-Besse event. As a result, IE Bulletin 85-03, " Motor-Operated Valve Comon Mode Failures During Plant Transients Due to Improper Switch Settings," was prepared and issued on November 15, 1985.

Adequacy of emergency notifications.
Review the adequacy of NRC guidance for determination of severity (b) 1evels when plant conditions vary and may be stable when the licensee l
has an opportunity to report.
IE Action: On October 15, 1985, IE Information Notice 85-80, " Timely Declaration of an Emergency Class, Implementation of an Emergency Plan, and Emergency Notifications," was issued to clarify the requirement for licensees to adequately notify the NRC Headquarters Operations Officer of emergencies.

Victor Stello, Jr. g2 7 1986 (c) Review the adequacy of shift staffing for assuring that knowledgeable individuals will be available for properly implementing the emergency plan during complex and long operational events.
IE Action: Personnel from IE and NRR reviewed NUREG-1154 titled " Loss of Main and Auxiliary Feedwater Event at the Davis-Besse plant on June 9, 1985." Based on this review and the analysis that supported the current shift staffing requirements, we do not believe that it is necessary or desirable to require additional personnel on shift to implement the emergency plan. The potential emergency planning problem raised by the June 9 Davis-Besse event is a command and control / training problem. The issuance of IN 85-80 by DEPER addresses the specific corrective actions undertaken at Davis-Besse and provides generic recomendations regarding the qualifications of personnel assigned to (1) assist the shift supervisor in classifying an emergency and activating an emergene planand(2)makenotificationstoNRC.

IE Action: We have detemined that an IE Infomation Notice is warranted. The information notice has been drafted and is in the concurrence process. The notice deals with the underlying cause of the Davis-Besse AFW turbine overspeed trip and with similar events that have occurred recently at other nuclear plants.

In addition to the actions described in Mr. Dircks' August 5,1985 memo, a teleconference meeting with the Office Director and Mr. Stello on January 16, 1986, resulted in the following IE action plan additions:

Consider issuing an information notice to provide guidance on the use of locks versus tamper seals on equipment.
IE Action: Consideration will be coordinated between IE, NRR, and NMSS with a target of 3/31/86 for issuance or other disposition, as appropriate.
