SUBJECT: REPORT NOS. 50-327/86-38 AND 50-328/86-38
On June 9'- 20, 1986, the NRC staff conducted a special inspection to examine
'
the Design Baseline and Verification Program (DBVP), including independent
oversight by TVA Engineering Assurance, for the Sequoyah Nuclear Power Plant.
The NRC inspection comprised a multi-discipline team of personnel from the
,
Office of Inspection and Enforcement, consultants, and the Region II Office.

The team concentrated its inspection on (1) the overall DBVP plan and scope;
(2) the implementing procedures for both the project and the associated
independent Engineerin

the walkdowns; and (4)g Assurance (EA) oversight;(3) conduct and results of
a preliminary assessment of the preparation of design
criteria.

As a result of the inspection, the NRC team concluded that the preparations
and approach for the program were in most cases in accordance with the DBVP
'
plan, subject to the items contained in the enclosed inspection report. These
items were provided to TVA in an exit meeting on June 20, 1986. We expect these
f tems will be addressed in your ongoing DBVP. We intend to review your actions
~
in response to these items in future inspections.

The following summarizes the major concerns identified by the inspection team
with respect to the DBVP.
(1) Several weaknesses in implementation of the DBVP were pointed out to TVA.
'
Examples include:
Inconsistencies between Units' I and 2 walkdowns, stemming from
excessive latitude given to syst+ms engineers on attributes to be
included.
Lack of verification of portions of system drawings where portions of

the system appeared on more than one drawing.

Failure to document decisions regarding walkdown limits being
,
less inclusive than program boundaries.

(2) The scope of the walkdowns does not include physical verification of
instrumentation and electrical aspects, such as conformance of system
functionality to single-line diagrams, cable separation, etc. TVA intends to
assess the adequacy of these aspects by review of test or surveillance
documentation or other controlled documentation which is available. The
rationale to support TVA's position needs to be documented.
(3) TVA is developing procedures and criteria as the program progresses, which
may result in accommodation of the procedures to the actual work performed.
(4) The Engineering Assurance (EA) group's review of the DBVP lacks guidance in
determining which review products should be sampled.
The EA review should
be focused towards (1) dominant safety systems, (2) complex Engineering
Change Notices (ECNs), and (3) areas where there is a known history of
industry problems.
TVA has decided' that the scope of the DBVP required to be completed prior to
restart will include those systems, or portions of systems, needed to mitigate
those accidents described in FSAR Chapter 15 or needed to achieve safe shutdown.
Although the team identified several specific comments relating to the program's
scope, the adequacy-of the program's scope is still undergoing staff review.
This letter also documents (report attachment B) several meetings which were
held prior to the inspection during which TVA discussed the DBVP in general
and information necessary for inspection planning was obtained.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosure
will be placed in the NRC Public Document Room.
