Gentlemen:

SUBJECT: NOTICE OF VIOLATION

(INSPECTION REPORT NOS. 50-259/86-40, 50-260/86-40 AND 50-296/86-40)

This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by

G. L. Paulk on November 1 - December 31, 1986. The inspection included a review

of activities authorized for your Browns Ferry facility. At the conclusion of

the inspection, the findings were discussed with those members of your staff

identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report. Within these

areas, the inspection consisted of operational safety, maintenance observation,

surveillance testing observation, reportable occurrences, licensee action on

previous enforcement matters, management meetings, design changes and modifi-

cations, and facility modifications, and the new employee concern program.

The inspection findings indicate certain activities appeared to violate NRC

requirements. The violation, references to pertinent requirements, and elements

to be included in your response are described in the enclosed Notice of

Violation.

Your attention is invited to unresolved items identified in the inspection

report. These matters will be pursued during future inspections.

Licensee event report (LER) 50-259/8606 Revision 1, was based on a probabilistic

risk assessment (PRA) which you consider an unreviewed draf t from which no

meaningful conclusions can be drawn. We request that you submit a revision to

this LER so that we can complete our review. The inclusion of unreviewed and

draft information is unacceptable for our purpose.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2, Title

10, Code of Federal Regulations, a copy of this letter and its enclosures will be

placed in the NRC Public Document Room.
