Subject: Inspection No. 50-293/87-04

This refers to the routine safety inspection conducted by Mr. N. Blumberg of

this office on January 12-16, 1%7, of activities authorized by NRC License No.

OPR-35.at Pilgrim Nuclear Power Station, Plymouth, Massachusetts and to the

discussions of our findings held by Mr. N. Blumberg with Mr. S. Hudson and

other members of your staff at the conclusion of the inspection, and to a

subsequent telephone discussion between Mr. E. Graham of your staff and Mr.

Blumberg on January 23, 1987.

Areas examined during this inspection are described in the NRC Region I

Inspection Report which is enclosed with tiin letter. Within these areas, the

inspection consisted of selective examinations of procedures and representative

records, interviews with personnel, and observations by the inspector.

Based on the results of this inspection, it appears that certain of your

activities were not conducted in full compliance with NRC requirements, as set

forth in the Notice of Violation, enclosed herewith as Appendix A. These

violations have been categorized by severity level in accordance with the

" General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR

Part 2, Appendix C (Enforcement Policy). You are required to respond to this

letter and in preparing your response, you should follow the instructions in

Appendix A.

Tais inspection identified four violations in the functional area of testing

programs and implementation. Beyond the specific problems identified by the

violations, we are concerned about the attitudes reflected by plant workers and

by the lack of initiative by plant management. In two of the violations plant

personnel recognized the problems but took no corrective action nor informed

plant management. The other two violations reflect lack of initiative by plant

management. Previous inspections have identified inadequacies in your test

procedures and the incomplete evaluation of your programs against regulatory

requirements; yet, corrective actions continue to be delayed. In your response

to these violations, we request that you address the personnel and management (

concerns we have identified above.
