Gentlemen:

Subject: Inspection Report No. 50-277/86-25

This refers to the special operational safety inspection conducted by a team of

NRC personnel led by Dr. Lee Bettenhausen of this office on December 8-19, 1986,

at the Peach Bottom Atomic Power Station, Unit 2, Delta, Pennsylvania, of activi-

ties authorized by NRC License No. DPR-44 and to the discussions of our findings

held by Dr. Bettenhausen with members of your staff at the conclusion of the

inspection.

Areas examined during this inspection included systems selected from the accident

sequences described in the Reference Plant Accident Sequence Likelihood Characteri-

zation: Peach Bottom, Unit 2. The inspection focused on the availability of

critical components as evidenced by plant conditions and implementation of programs

to assure operability. In addition, the inspection examined the ability of the

Peach Bottom Unit 2 staff to utilize procedures and equipment to recover from

certain postulated accident sequences. These areas are described in the NRC

Region I Inspection Report which is enclosed with this letter.

Within these areas, the inspection consisted of selective examinations of proce-

dures and representative records, interviews with personnel, and observation of

equipment and simulated activities by the inspector.

Based on the results of this inspection, it appears that certain of your activi-

ties were not conducted in full compliance with NRC requirements, as set forth

in the Notice of Violation, enclosed herewith as Appendix A. These violations

have been categorized by severity level in accordance with the General Statement

of Policy and Procedure for NRC Enforcement Actions, 10 CFR Part 2, Appendix C

(Enforcement Policy 1986). You are required to respond to this letter and in

preparing your response, you should follow the instructions in Appendix A. A

deviation from commitments in your Final Safaty Analysis Report is identified

in Appendix B. You are requested to respont to this deviation.

In addition, the team identified weaknesses certain areas that warrant atten-

tion. These include: 1) updating emergenc, perating procedures; and resolving

human factors problems; 2) completely and consistently labelling plant equipment

and insuring correct procedural identification; 3) providing complete technical

information such as AC and DC circuit load lists; and 4) maintaining and testing -

supporting equipment. In your response to this report, please include the actions

you have taken or plan to take to address these concerns.
