50-400/86-89-01 Reported Violation:
10CFR-Part 50, Appendix.B, Criterion II and the accepted Quality Assurance Program (Final Safety Analysis Report, Section 17.2.2) collectively require that activities affecting quality shall be accomplished under suitably controlled conditions. Controlled conditions include the use of appropriate equipment and suitable environmental conditions for accomplishing the activity, such as adequate cleanliness. The Shearon Harris Nuclear Power Plant (SHNPP) FSAR Section 1.8 commits to RC 1.39 (Rev. 2),

Contrary to the above, on November 17 and 18 the licensee failed l
to maintain a record of entry and exit of all personnel and material in a Class B cleanliness zone which is the designated cleanliness area around the open reactor vessel.
This is a Severity Level IV violation (Supplement I).

exiting the fuel loading area is considered to be the root cause of the violation.'

Corrective Steps Taken and Results Achieved:
I Upon discovery that the access and material controin in place during initial fuel loading were not adequate fuel movement was

halted. A management directive was issued to establish adequate access and material control. The directive was issued at 1430 and was fully implemented by 1545 November 19, 1986. After implementation of the directive fuel movement resumed.
These controls remained in place until fuel loading was completed and the reactor head was replaced.
Corrective Steps Taken to Avoid Further Noncompliance:
A permanent plant procedure (TMM-112) was approved on December 31, 1986 which incorporates and amplifies those controls put in place during initial fuel load for future fueling evolutions.
