Subject:

Inspection No. 070-02895/88-01 This refers to your letter dated June 14, 1988, in response to our letter dated April 7, 1988.

With regard to Items A.1 and A.2, in the attachment to our letter, your license requires that a bioassay (i.e. urinalysis) be performed every six months and that swipe surveys be performed every month.

Your reply indicates that you desire to change this frequency.

Considering the type, form and quantity of material, it may be possible to grant this change. However, in order for us to review this change a formal request for a license amendment must be sent to this office accompanied by the required fee. The person to contact regarding the correct fee is Sandra Kimberly at (301) 492-8743.

With regard to Item A.3, in the attachment to our letter you state that a consultant came to your facility to provide training, but do not state whether a survey was performed.

Please state whether the consultant performed a survey of your facility as required by your license.

If you desire to change the required frequency of the consultant's surveys, you must also request that your license be amended.

With regard to item B, in the attachment to our letter no corrective action is described for the fact that you are presently operating without any authorized users listed on the license.

You must request that your license be amended to delete the named individuals who are no longer supervising the use of material and to add the individuals presently supervising the use of material.

In order to do this submit a formal request describing their trainir) and experience with radioactive material. All of the above items may be included in the same request to arrend your license.

A reply to this letter providing the requested information is required within 30 days of your receipt of this letter.

Failure to respond to this letter may result in additional enforcement action.

You should also promptly file a formal request to amend your license.
