Routine Inspection No.88-001 On January 21, 1988, Steven Courtemanche of this offico conducte ' a routine ;
safety inspection at the above address of activities authorized L. the above ;
listed NRC license. The inspection was an examination of your licensed activi- l ties as they relate to radiation safety and to compliance with the Commission's '
regulations and the license conditions. The inspection consisted of observations '
by the inspector, interviews with personnel, and a selective examination of i J representative records. The findings of the inspection were discussed with yourself at the conclusion of the inspection.
Our letter dated February 12, 1986 transmitted a Notice of Violat on (NOV).
i You were requested to respond within thirty days of the receipt of that letter, ,
No response has been received in this office. On January 21, 1988, the inspector reviewed the actions taken by you to correct the violations identified i in that NOV. We have no further questions regarding Item A.2. However, as discussed below, items A.1 and B. have not been corrected.
Br. sed on the results of this inspection, it appears that your activities were t not conducted in full compliance with NRC requirements. A Notice of Violation I is enclosed as Appendix A and categorizes each violation by severity level in accordance with the "General Statement of Policy and Procedure for NRC Enforce-ment Actions," 10 CFR Part 2, Appendix C (Enforcement Policy). You are required to respond to this letter and in preparing your response, you should follow the instructions in Appendix A.

identified during a previous inspection of your licensed program and documented l in a Notice of Violation enclosed with our letter dated February 12, 1986.
From this inspection it appears that your corrective actions were not effective ;
since these items have recurred. Recurrent and uncorrected violations are ;
gi'.'en additional weight in the consideration and selection of appropt late '
enforcement action. Therefore, in your response to this letter, you should give particular attention to those actions taken or planned to ensure that .
identified items of noncompliance will be completely corrected and will not l recur. Failure to respond to this letter may result in additional enforcement action.
