Gentlemen:

Subject: Inspection No. 50-29/86-09

This refers to the special inspection conducted by Mr. H. Eichenholz of this of-

fice on June 26 - July 2, 1986, at the Yankee Nuclear Power Station, Rowe, Massa-

chusetts.

The inspection was performed as a result of your discovery on June 26,

1986 that the Reactor Coolant System Vents (RCSV) and Emergency Feedwater (EFW)

System motor operated valves were inoperable.

The inspection findings were dis-

cussed with Mr. N. St. Laurent of your staff at the conclusion of the inspection.

Our review of the circumstances associated with the identified deficiencies indi-

cates that the RCSV and the EFW System were inoperable from November 27, 1985 until

June 26, 1986 due to the installation of undersized trip coils in the combination

breaker / starters for valves EBF-MOV-557, PR-MOV-558, VD-MOV-559, and PR-MOV-560.

We are concerned that (1) your procurement and post-installation testing programs

did not detect the inclusion of an incorrect device that affected the operability

of these safety systems, and (2) that this specific issue may be symptomatic of

a broader problem.

As a related matter, we are also concerned that you apparently

have not provided sufficient resources and training to site engineering personnel

involved with the above programs as recommended in our most recent SALP report.

The apparent violation identified in our review of this event is discussed in the

enclosed report. The need for, and nature of, appropriate enforcement action rela-

tive to the apparent violation will be the subject of separate correspondence at

a later time. These matters will be discussed in an enforcement conference with

you and members of your staff scheduled for July 22, 1986 at 9:00 a.m. in the

Region I office. You should come to the meeting prepared to discuss your under-

standing of these events, the reasons that they occurred, and your planned and

completed corrective actions.
