Balta" 7 x , ~ ~ '" c %-My 2.0 PURPOSE AND SCOPE This instruction provides the inspection criteria and documenta-tion requiremehts used to perform "as-built" verification of Seismic Category I cable tray supports and clamps supporting tray designated as Unit 1 in all Category 1 Buildings. NOTE: The main structural steel framework in Room #133 (Cable Spread Room) is excluded from the scope of this instruction. The QC Inspector shall perform inspection of the cable tray hanger to verify that the drawing created by engineering depicts actual field conditions of certain specified attributes of the cable tray hanger. Any weld, dimension or attribute identified on the preliminary "as-built" drawing by Engineering as "IA" 'v') shall be verified by the QC Inspector as inaccessible. Inspec-t tion shall only be performed on attributes accessible for inspec-tion. Bus Duct - Where criteria in this instruction or CTH drawings refer to " cable tray", the criteria shall also be used for " Bus Duct" when applicable. 3.0 INSTRUCTION 3.1 USE OF WORK PACKAGE FOR CTH "AS-BUILT" VERIFICATION The PFG (Package Flow Group) shall issue an individual work package for each cable tray hanger. 3.2 USE OF PRELIMINARY AND FINAL "AS-BUILT" DRAWING 3.2.1 Use of the Inspection Report and the Attached Drawing Each package shall contain a prenumbered inspection report and a copy of the preliminary as-built drawing to be used as an attachment to the inspection report. The copy of the drawing shall be identified with the inspection report and page numbers, then signed, dated and attached to the body of the inspection report whether unsatisfactory conditions exist or not. Signing of the copy of the preliminary "as-built" drawing shall indicate acceptance of all dimensions, attributes and welds shown on the drawing which are not listed as unsatisfactory on the inspection report. l NOTE: Clamp gaps, if shown on the preliminary "as-built" l drawing are not to be verified, gaps shall be as shown on Attachment 11/11A of this instruction. i Normally only one copy of the drawing shall be used to document all unsat conditions (welding, dimensions, configurations, etc.), however, an additional drawing or attachments may be used if necessary. All drawing (s) and attachments shall be attached to the body of the inspection report. 3.2.1.1 "As-Built" Verification of Dimensions and Attributes On the preliminary "as-built" drawing each dimension and/or attribute shown shall be checked for compliance, using the tolerances listed on Attachment 2. Inspection results shall be documented by checking the applicable block (sat, unsat or N/A) for each pre-determined attribute on l l..-

When unsatisfactory conditions exist, they shall be clearly described under each applicable attribute with the location of unsatisfactory conditions indicated on the attached drawing when practical using a designator to provide traceability. 3.2.1.2 " As-Built" Inspection of Welds Support welding shall be inspected for qualitative and quantitative attributes as outlined in Paragraph 3.3.5 of this instruction. Wald inspection results shall be documented by checking the applicable block (Sat, Unsat or N/A) on the inspection report (Attachment 1). The Inspector shall verify that each weld documented as unsatisfactory on the body of the inspection report is independently traceable on the attached drawing. When a weld joint number encompasses more than one veld, and one or more of the welds are unsatisfactory, the inspector will renumber the welds on the attached drawing with alpha-numeric designators to provide traceability to each individual weld on the. inspection report. Any additional welds not indicated on the preliminary "as-built" drawing will be added to the copy of the preliminary "as-built" drawing and identified with the next higher joint sequential number code. The additional welds by number will be documented as unsatisfactory NOTE: Fillet welds shall be considered inaccessible only when neither leg can be measured. When only one leg is accessible for measurement, the drawing will have an asterisk adjacent to the fillet veld size and an arrow pointing to the member on which the accessible leg rests. 3.2.1.3 a. Verification of Inaccessible Areas Verify all inaccessible field conditions are correctly depicted on the preliminary as-built drawing. Inspection results shall be documented on Attachment 1 Attribute 12, except welding which will be documented on Attachment 1 O Attribute 5. 1 I

Resolution of Inaccessible Items Recuired for Design Review l Information identified and verified by QC on the preliminary "as-built" drawing as inaccessible that is determined by engineering to be essential for design review shall be resolved by engineering generating an " Inaccessible Item Resolution Traveler". On the traveler each operation will have an " Inaccessible Item" block for engineering to list the inaccessible item and a "QC Resolution" block for QC to explain how the item was resolved. Items will be resolved in one of the following methods: 1. QC/QE will do a review of historical documentation to determine if that item has been previously accepted. If historical documentation exists and is specific, QC shall close that step on the traveler and list all (~' applicable historical documentation used for acceptance L,/ in the space provided for QC resolution. 2. If craft renders an item accessible for inspection, QC will perform a field verification, record pertinent information in the space provided for QC resolution, and close the step on the traveler. 3. When no historical documentation exists and craft is not able to render the item accessible for field verification without major rework, QC will document the situation on an NCR. 3.2.2 Final "As-Built" Verification" Verification of the final "as-built" drawing shall be accomplished by one of the following three cases. Case 1. After initial inspection, if the support requires no modification after design verification, the copy of the preliminary "as-built" drawing which is now part of the inspection report (Attachment 1) and the final "as-built" drawing shall be compared to verify that all information, including unsatisf actory conditions noted that required no modification, were accurately transferred to the final "as-built" drawing. Results 7s of final "as-built" drawing verification shall be (v) documented on Attachment #4.

For supports having unsatisfactory conditions which require rework, craft will receive the package and correct unsatisfactory conditions. Unsatisfactory conditions will be reinspected and if found acceptable the inspection report closed. The final "as-built" drawings shall be reviewed as outlined in case 1. Case 3. Engineering will release the " Approved for Construction" (AFC) drawing for supports requiring modification. Craft will complete the modifications required and correct any outstanding unsatisfactory conditions as outlined in Case 2. QC verification of required modification will be performed using a " construction operational traveler". The QC Inspector shall review the final "as-built" drawing to verify the following: a. All applicable dimensions and attributes shown on the copy of the preliminary "as-built" drawings which are now part of the inspection report (Attachment 1), have been incorporated. b. All required modifications as shown on the AFC drawing have been implemented and are acceptable. c. All unsatisfactory inspection reports have been closed. d. All modifications as shown on the AFC drawing have been incorporated on the final "as-built" drawing. Results of final "as-built" drawing verification shall be documented on Attachment #4. NOTE: An item shall be considered deleted if the dimension or material size is no longer indicated on the final "as-built" drawing. 4 3.3 SUPPORT INSTALLATION VERIFICATION 3.3.1 Cable Tray Support Identification Prior to inspection / verification, the QC Inspector shall verify that the cable tray support has been identified by Site Field i Engineering. Identification shall be by a stainless steel tag with the CTH number permanently stamped on it, and the tag affixed to the CTH with a plastic tie wrap. In the Containment s Building, the plastic tie wrap shall be "Tefzel". .r - - ~

NOTE 2: The unit designation number may or may not be included on the stainless steel identification tag attached to the support. 3.3.1.1 Cable Tray Support Location and Elevation Cable tray support location, elevation, spans, plumb and level-ness are verified by field engineering and are not within the scope of this procedure. 3.3.2 Bolting Inspection 3.3.2.1 General Bolting Requirements For each support the following shall be verified by the (G inspector, if applicable as denoted in paragraph 3.3.2.2 ) (Specific Bolting Requirements). a.
- Diameter, b.
Spacing and location (dimension verification fror centerline of bolt), c. Projection from concrete surface and letter identification (Hilti Bolts), d. Material type and grade when required by design for bolts and washers, if bolt type is specified as other than A-307 - (i.e., A-325 A-490, A-193 gr. b-7 etc.). Verification of nut material is not required unless a friction type connection is specifically noted on the drawing (i.e., A-325-F). See Attachment 6. e. Engagement (face of nut minimum of flush with top of bolt), f. Related bolting material (excluding nuts). Hardened washer verification is only required if the preliminary "as-built" drawing specifies the use of ASTM A490 type bolts. g. Proper bearing contact, bolt heads and nuts resting against the metal or washer with a slope of 1-in-20 or less shall be considered acceptable.

Where bolts are used on surfaces having slopes greater than 1-in-20 with a plane normal to the bolt axis, beveled washers shall be provided. Installation of beveled washers shall be such that the beveled surface of the washer bears against the sloping surface of the member. An irregular gap of 1/16" max, is acceptable between the washer (beveled and flat) and the surface of the member. Caps between bolt heads and nuts and washers are subject to the provision of g above. i. Quantity as shown on the pre 10minary "as-built" drawing. 3.3.2.2 Specific Bolting Requirements' i 3.3.2.2.1 Hilti Bolts f -Hilti bolts and/or Hilti Super Kwik bolts shall be inspected for projection, size, type, bearing contact, identification, ' / ') engagement, washer installation, spacing / separation, presence of \\m / torque seal and location as shown on the preliminary "as-built" drawing and Attachment 5. When letter identification and star stamping is not readily visible, it may be verified by ultra-sonic testing per Reference 1-A or 1-G and af ter verification is e complete the bolt shall be stamped with the correct letter and l star stamp as applicable. If torque seal is broken or not present, the bolt shall be documented as unsatisfactory on 4. 1 NOTE 1: Torque seal is only required on a minimum of one bolt per base member, if more than one bolt has torque seal, it must be intact. NOTE 2 Hilti nuts may be installed with or without the washer face against the washer. 3.3.2.2.2 Richmond Inserts (Screv Anchors) All bolts used for this installation shall be inspected for size, spacing / separation, engagement (minimum 2d + 1/8"), presence of torque seal, location bearing contact and material type shown on preliminary "as-built" drawing and Attachment 5. Engagement may j be verified using Ultrasonic Testing per Reference 1-B or 1-F. l Richmond insert diameter may be verified by measuring the head of the bolt across the flats to determine the corresponding diameter as follows; the bolt head width of a one inch bolt is O approximately 1 5/8", the bolt head width of a one and one half inch bolt is approximately 2 3/8". When all thread rod is used it may be identified on the preliminary "as-built" drawing as " doubled nuts" or "dn" and shall be considered as A-36 material (no material or identification verification required) unless denoted otherwise on the preliminary "as-built" drawing. If torque seal is broken or not present, the bolt shall be l documented as unsatisfactory on Attachment #1. Washers are not required but may be used. NOTE: Torque seal is required on all richmond insert bolts. 3.3.2.2.3 Structural Bolts f l All structural bolts and related hardware shall be inspected for size, spacing, material type, engagement and bearing contact as shown on preliminary "as-built" drawing. t The QC Inspector shall verify that nuts on structural bolts O exhibit no looseness when checked by hand only. ASTM A-449 bolts may be substituted for A-325. 3.3.3 Verification of Supporting Member for Bearing Cable tray hanger base members (base plates, base angles, etc.) shall be inspected for bearing between concrete surfaces and support base. Bearing is acceptable if any visible gap is 1/16" or less for a 5" x 5" envelope around the bolts. The use of permanent shims to prevent base member distortion during torquing is acceptable. However, if shims are used the base member shall be grouted. i If the gap is greater than 1/16 or shims were used the base member shall be grouted per existing site procedures. Document i unsat bearing conditions on Attachment 1. l NOTE 1: Base members which have been previously grouted by the use of epoxy or cement grout are acceptable. l NOTE 2: Tack welds on shims shall be considered Non-Nuclear Safety Related and need not be inspected except for base metal damage, if base metal damage (including undercut) exists as a result of the welding process, i which exceeds the limits of Attachment 10, it shall be documented as unsatisfactory on Attachment 9. Dimensions shown shall be verified to be within tolerances given on Attachment 2. Significant unit of measurement shall be 1/16". NOTE: Dimensions shown to depict thermolag and/or fire blankets shall not be verified by QC, b. Support member shape (i.e., angle, channel, etc.), size and orientation. Significant unit of measure shall be 1/16" ( except for' member thickness which shall be 1/32". ( For standard members, the size shall be determined (using
- ) by the member designation closest to matching the dimensions measured in the field. When member size's cannot be readily determined because dimensions fall between two sizes, the lower weight member shall be selected.
l NOTE: In some cases Engineering may provide a cross } sectional view of members that cannot be readily identified. In this case, the QC inspector will verify the as-built member dimensions, and may not be required to determine member size. c. Tube steel or any closed section members used inside the containment having cover plates (cap plates) must have vent hole (s) as shown on the preliminary "as built" drawing. If vent holes are not present, QC shall document as unsatisfac-tory on Attachment 1. d. All drawing sections, plans and views are in agreement with the drawing key plan. e. The cable tray hanger drawing will contain the as-built information for the specific cable tray hanger number designated in the drawings. In several instances other cable tray hangers connected to the same base angle or \\ base / embedded plate may be shown " ghosted" in the same drawing. Page 11 of 70 / - CPSES - JUL 011986 The example of Attachment 12 illustrates the inspection process to be applied in those instances. If the inspector is inspecting CTH-XXX, he should not inspect and verify the attributes associated with the base angle (shown ghosted) or CTH-YYY. This means that no reinspection or verification of the anchorages, bearing area, etc., will be conducted. That inspection verification will be done when CTH-YYY is inspection and verified with its own drawings. 3.3.5 Welding Inspection 3.3.5.1 Procedure Requirement Welding inspection shall be in accordance with Reference 1-E, VWAC (NCIG-01) criteria except as noted below. When welding is designated as "NNS" (i.e., Non-Nuclear Safety) it shall not be inspected except for base metal damage. If base ,- s ( ) metal damage (including undercut) exists, it shall be inspected and reported as outlined in 3.3.3, Note 2. The welds at the ends (or corners) of angles, channels and vide flanges where an all-around fillet weld has been detailed on the individual support drawing are optional and non-safety related. 3.3.5.2 Full and Partial Penetration Welds All full penetration welds and partial penetration welds exclud-ing structural members forming a natural groove as specified in Reference 1-E shall be documented on an NCR in accordance with Reference 1-C with the erceptions listed in Note #1. NOTE 1: For clamps an NCR will not be required if the fit-up preparation is visible on the unwelded portion of the plate, and shall be considered as having acceptable penetration when the unwelded portion of the member / plate is beveled as required (45*, +10*, -20*). However, if lack of penetration is evident, it shall be documented on an NCR as outlined above. NOTE 2: The QC inspector shall inspect all structural members for evidence of a splice. All members found to have a splice shall be documented on an NCR in accordance with Reference 1-C, regardless of whether the weld is shown / ) on the drawing or not. \\,__/
h TUGC0 INSTRUCTION l QI-QP-11.10-9 Rev. 5 UXAS UTILITIES GENERATING CO. p,. ) Page 12 of 70 - CPSES - JUL 011986 3.3.5.3 Attachment to Embedded Plates The QC Inspector shall verify that any welded attachments to embedded plates are located in acceptable areas as shown on and Attachment 5 if specific dimensions are not depicted on the preliminary "as-built" drawing. 3.4 BASE METAL DAMAGE / DEFECTS 3.4.1 Misdrilled Holes The QC Inspector shall use the following criteria to assess the need for repair of misdrilled holes: Cable Tray Support a. All unused holes greater than 3/4 inch diameter-shall be s

Unused holes which are spaced 2 inches or less (measured center to center) from a used or unused hole shall be
- repaired, c.
No two holes (used or unused) shall be located in the same cross-sectional plane (see Attachment 8 for typical examples of acceptable and rejectable bolt / hole configurations). In summary, when a used hole and an unused hole are in the same cross-sectional plane, the unused hole shall be repaired. Conversely, when two (2) unused holes are in the same cross-sectional plane, one must be repaired. These configurations shall be documented on an inspection report (Attachment 9). A cross-section is a section perpendicular to the longitudinal axis of the member. Cable Tray For cable tray, each siderail shall be evaluated indepen-dently. See Attachment 8 for typical examples of acceptable and rejectable bolt / hole configuration. These configura-tions shall be documented on an inspection report (Attach-ment 9). NOTE: If cable tray clamp installation results in holes j in the cable tray that violate the requirements of s_,/ this paragraph, they shall be identified in the same manner as outlined above.
} TUGC0 INSTRUCTION V!AS UTILITIES GENERATING CO. g> q QI-QP-11.10-9 Rev. 5 Page 13 of 70 j. - CPSES - JUL 011986 Repair of the above misdrilled holes shall be inspected per Reference 1-E. 3.4.2 Other Base Metal Damage / Defects Other base metal damage / defects noted during "as-built" inspection / verification shall not exceed dimensions / area as shown on Attachment 10. Base metal damage / defect shall be documented on an inspection report (Attachment 9). Repair of base metal damage / defect shall be inspected per Reference 1-E. NOTE 1: Removal of paint for identification of base metal defect / damage is not required. NOTE 2: This paragraph does not apply to undercut except as noted in paragraph 3.3.3, Note 2 and paragraph 3.3.5.1. 3.5 CABLE TRAY CLAMP INSTALLATION 4 3.5.1 Bolting Inspection Bolted cable tra'y clamps will be verified per paragraph 3.3.2., 1 and the preliminary "as-built" drawing. The correct orientation of wedge (bevel) washers if used and/or j required will also be verified (per Attachment llA). 3.5.2 Welded Cable Tray Clamps l Welded cable tray clamps will be verified per Attachment 11, the i preliminary "as-built" drawing and paragraph 3.2.2 and paragraph 3.3.5. 3.5.3 Verification of Cable Tray clamp Installation Cable tray clamps shall be verified for proper installation per 1 and/or preliminary "as-built" drawing. NOTE 1: When Attachment 11 or 11A states " verify presence" for a component, it shall be understood that no dimensional verification is required and only its existence should be verified. NOTE 2: Clamp installation gaps shall be inspected to verify that they meet requirements of Attachment 11/llA. Field F'bricated Clamps 3.5.4 a The inspector shall verify that field fabricated clamps meet dimensional requirements as shown on Attachment 11.

Type A & C clamps which are not hot dip galvanized are field fabricated. 3.6 CABLE TRAY The QC Inspector shall verify that cable tray nominal width, nominal depth, location and orientation are as shown on the preliminary "as-built" drawing. 3.7 ADDITIONAL ATTACHMENT TO CABLE TRAY HANGER Additional attachment to the cable tray hanger (i.e. conduit support, pipe support, etc.) shall be verified as shown on the preliminary "as-built" drawing. Attachment of thermocouples (fire sensor wires), and 1/2" grounding cables to the CTH shall not be considered as an additional attachment. O 3.8 DOCUMENTATION 3.8.1 Document Review The cable tray hanger Quality Control Supervisor or his designee shsil review and complete the cable tray hanger package review checklist (Attachment 13) in accordance with the inspection requirements of this procedure. NOTE: The cable tray hanger package review checklist (Attachment 13) shall become part of the cable tray hanger package upon completion. However, the checklist may be disgarded after package corrections have been accomplished. 3.8.2 Inspection Reports Inspection results, except as outlined in para. 3.4.1 and 3.4.2 shall be documented on Attachment I and 4. CTH drawings are identified with a unit designation number CTH-1-XXX. This unit designation number may or may not be included in the identification number on inspection reports. All inspection reports shall be processed in accordance with Reference 1-D.

- 2. 1 specifies those clamp details and dimensions which are for inspection purposes. Dimensions not given on the preliminary "as-built" drawing, such as gaps, dimensions of field fabricated Burndy Husky clamps, etc., shall be as shown in Attachment 11.
For "special" clamps not covered in Attachment 11, details and dimensions shall be as shown on the preliminary "as-built" drawing. Cap require-ments given in Attachment 11A override any clamp gap requirement shown on the preliminary "as-built" drawing. For clamp types specified on the CTH drawing as other than type A, C, G and "special", they shall be inspected as a one bolt, two bolt or multi bolt clamp configuration O as applicable. 3. Tray clamps shown in this attachment may be attached to a supporting tube steel, angle, plate, channel, section, walls or floors as applic-able and as shown on the preliminary "as-built" drawing. If a shim / filler / spacer plate is welded to both the clamp member and the structural member, the plate is considered part of the structural member. In this case any welding unless specifically called NNS on the CTH drawing is considered structural and the weld is safety related. 4. Washer plates on one or two bolt clamp details of the connection to the tray of 24" and wider are required, (verify presence) washer plates may or may not be used on tray less than 24". Where A307 round head bolts with standard washers are specified, the conventional symboI on Attachment 11 and on preliminary "as-built" drawing should not be construed to represent the location of the washer. The washer shall be installed under the nut. Flat washers may be combined with flat or bevel washers. No more than two washers shall be stacked. When a flat and bevel washer are stacked, either order of stacking is acceptable. Flat washers may be installed under either or both the head of the bolt and the nut; however, the existence and location of flat or bevel washers is not required to be shown on the preliminary as-built drawing. 5. If the irregular gap between the tray and the clamp exceed 3/16, l f-sg ( ) washer plates shall be provided. In case washer plates are used the \\- maximum irregular gap of 3/16 is to be measured as a total gap between clamp and tray, less the thickness of the washer plate (s) (verify presence).
