Subject: NRC Inspection San Onofre Unit 3
This refers to the inspection conducted by Messrs. H. S. North, J. E. Russell,
and G. P. Yuhas of this office on December 15, 1986, January 12-16, and
March 16-20, 1987, of activities authorized by NRC License No. NPF-15 and to
the discussion of our findings held by Mr. Yuhas with Messrs. Ray and Morgan
on March 20, 1987, and other members of your staff at the conclusion of the
inspection.
Areas examined during this inspection are described in the enclosed
inspection report. Within these areas, the inspection consisted of selective
examinations of procedures and representative records,' interviews with
,
personnel, and observations by the inspector.
Based on the results of this inspection, it appears that you failed to
adequately control licensed radioactive material as necessary.to ensure that

personnel are not exposed in excess of the regulatory limits. Enforcement
,
acthns relative to the findings discussed in the enclosed report will be the
subject of separate correspondence.

The results of this inspection were reviewed with you, Mr. Ray and other
members of your staff in our offices on April 10, 1987. Based on this
meeting it is our understanding that you may submit a supplement to LER
86-015 that would address the issues discussed at the meeting, including an
evaluation of the health effects of the subject extremity overexposure.
