Dear Mr. Rainey:

On November 9,1998,the NRC completed an inspection at the Peach Bottom Atomic
Power Station. . The. enclosed report presents the results of that inspection.
i
.
.Overall, your staff performed well during the 2R12 refueling outage. We determined that
!
' ALARA and radiological controls for outage ' work activities were effectively established and
i
!
implemented. We also noted very good performance by operations personnel throughout
the outage.
We were concerned during the inspection period with the failures of maintenance personnel
l
to adequately self-check or peer-check'their work.- Two notable examples included the
- inadvertent trip of the Unit 3 E33 bus that resulted in several automatic engineered safety
feature actuations and the mispositioning and misorientation errors'that occurred during the
refueling core alteration on Unit 2. Although operations personnel have made substantial
' improvements in self-checking and peer-checking, challenges still remain in these areas for
other station work groups, especially maintenance.
'
We also noted that your staff identified four examples of failures or degraded conditions on
safety related motor operated valves during the 2R12 outage. Although, the valves were
operable and capable of performing their safety function, when required; we were
concerned that the failures and degraded conditions indicated a negative trend in motor
!
operated valve reliability.

Based on the results'of this inspection, the NRC has' determined that one violation of NRC

' requirements occurred.- This violation is cited in the enclosed Notice of Violation (Notice)

report.

The violation occurred when chemistry technicians failed to stop and notify control room
personnel and Chemistry Supervision and request permission to continue a surveillance
test, when a radiation detector was inadvertently removed and dropped, damaging the
detector. We were concerned with this violation because the technicians did not
document in the Action Request or notify anyone that they had removed and dropped this
detector until Chemistry management started an investigation of the damaged detector.
The NRC has cc,ncluded that information regarding the reason for this violation, and the
actions taken or planned to correct this violation and prevent recurrence is already
adequately addressed in this inspection report. Therefore, you are not required to respond
to this violation unless the description in the referenced report does not accurately reflect
your corrective actions or your position. In that case, or if you choose to provide
additionalinformation, you should follow the instructions specified in the enclosed Notice.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter
and its enclosures will be placed in the NRC Public Document Room (PDR).
