Subject: AUGMENTED SYSTEMS REVIEW AND TEST PROGRAM INSPECTION

(50-312/86-41)

This letter forwards the report for the Augmented Systems Review and Test

Program (SRTP) inspection performed by an NRC inspection team over the period

December 1,1986 to February 12, 1987, involving activities authorized by NRC

Operating License No. DPR-54 for the Rancho Seco Nuclear Generating Station.

This inspection was conducted by members of the Office of Inspection and

Enforcement and NRC contractors. At the conclusion of the inspection, the

findings were discussed with you and members of your staff identified in

Appendix A of the enclosed inspection report.

In a letter to you dated

March 11,1987, we provided a preliminary summary of the more significant

inspection findings.

The NRC effort involved an assessment of the effectiveness of the SRTP process

dnd an evaluation of the adequacy of ongoing programs to ensure that systems

will continue to function properly after restart. This was accomplished by a

detailed review of the SRTP as implemented on a sample of 8 of the 32 systems

included in the SRTP and an in-depth review of your engineering and modification,

maintenance, operations, surveillance and inservice testing, ar.d quality programs.

This repcrt includes findings that may result in enforcement action, which

wuuld be the subject of subsequent correspondence. The report also addresses

other observations and conclusions made by the inspection team. Section 2 of

the report is a summary of the more significant findings. The detailed findings

are presented in Section 3.

The team was concerned that, although the problem identification phase of the

SRTP appeared to be gener611y effective, several examples were noted where

the resolution of the identified problems lacked sufficient engineering and

operations involvement. A notable example of the weaknesses identified in the

SRTP problem resolution phase was your failure to identify the fact that your

surveillance program did not adequately demonstrate that auxiliary feedwater

pumps would provide the flow required by Technical Specifications.

Further,

the team identified several deficiencies with the engineering analyses for

ongoing system modifications. These items, in addition to other significant
