HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING APRIL 10, 1987 Peach Bottom Nuclear Power Plant A special meeting of PECo's Board of Directors approved a plan to investigate Peach Bottom operations as a result of the NRC's Order of March 31, 1987 shutting down the two units. A multiple team approach will be used to assess the situation, implement corrective actions where necessary and develop a recovery plan. An in-house team, consisting of top corporate officers from PEco and the other joint owners has retained MAC of San Diego, CA to assist in the in-house investigation; MAC has had seven people on site since Thursday, April 2.

The second team, comprised of four of the Board of Directors, has retained Admiral Eugene R. Wilkinson as a consultant.

(Admiral Wilkinson is past president INPO.) The third team will be an independent analysis by INP0 of Peach Bottom operations. No schedule has as yet been developed by any of the teams for completing the investigations.

Seabrook Unit 1 On April 8, 1987, Mr. W. B. Derrickson, Senior Vice President Nuclear Energy, New Hampshire Yankee (NHY) Division of Public Service Company of New Hampshire informed the staff that they are delivering to the NRC today a utility emergency plan for the Massachusetts portion of the EPZ. The plan consists of 26 volumes. Additional copies of the plan will be transmitted to the parties.

NHY's transmittal letter will note that the Commission's action on ALAB 853 should be moot with the submittal of this plan.

Fort St. Vrain i

By confirmatory order, the Public Service Company of Colorado has been authorized to restart and operate the Fort St. Vrain HTGR at a level up

to 35% of full power. The facility has been out of operation since May 31, 1986, when it was shut down for equipment qualification modifications.

Current reactor core conditions will allow for a short " dry out" period.

After startup, the licensee will " hold" at 10% of full power to allow the staff to evaluate the facility's performance. Upon the staff's approval, the plant may operate up to 35% of full power.

Pending completion of additional licensing actions, the plant will be authorized to operate at 82% of full power.

Week Ending April 10, 1987 St. Lucie Plant, Unit No. 1 A number of. events occurred at the St. Lucie Plant Unit No. I while preparing to return to service after a refueling outage.

Unit I was in Mode 3 on Tuesday, March 31, 1987. Mode 3 for Unit 1 is T

greater than or equal to 325*F, Core Suberitical. The licensee d$lIrminedthattheinner"0"ringsealatthereactorheadtovessel flange joint was leaking. A visual inspection of the head circumferential area (insulation not removed) indicated no visible leakage. The leak rate thru the inner seal was estimated to be approximately 2.3 gallons per hour.

1 The licensee elected to cooldown the unit and replace the "0" rings.

On Wednesday, April 1, 1987, the licensee was cooling down the unit. Cooldown from hot standby to 325'F is accomplished through the steam dump'and bypass system in conjunction with either the feedwater or auxiliary feedwater system. The shutdown cooling system is then used for the remainder of the cooldown. The shutdown cooling system entry temperature and pressure is 325 F and 265 psi, respectively.

The r'eactor coolant system pressure was below 1750 psi but above 265 psi at the time. The shutdown cooling system was isolated from the RCS. The licensee was planning to heat up the shutdown cooling system. This is accomplished by opening a recirculation line from one of the LPSI headers back to the suction i

of LPSI pump 1A and also opening up a second recirculation line from a second j

LPSI recirculation header to the suction of LPSI pump 1B. These lines.have relief valves.

The licensee started LPSI pump 1A. The licensee attempted to start LPSI pump IB but it failed to start. The relief valve associated with suction line of LPSI pump 1B lifted. The 1A pump was subsequently shutdown.

The licensee is evaluating why the relief valve lifted. The licensee has also assembled a multi-discipline quality improvement program (QIP) team to i

investigate the cause of the pumps not starting.

Initial indicators suggest loose fuse holder clips. The pumps were eventually started and the unit was cooled down.

A regional inspector was sent to the site. A twenty day shutdown is ervisioned to remove the head and replace the "0" rings.

Unit 2 is at full rated power. All safety related pumps for Unit 2 were succersfully tested on April 2.

Palisades Plant Palisades plant went critical at 10:30 a.m., April 2, 1987. The plant has completed a 10 month maintenance outage during which many systems and components were repaired. The corrective work order back-log was reduced from 1305 to 697 work orders. The plant is scheduled for another two month maintenance outage in fall of 1987. The next refueling will be in late spring of 1988.

Oconee Nuclear Station, Units,1, 2, and 3 At Oconee Nuclear Station, Units 1, 2 and 3, the coolers of the low pressure injection (LPI) and reactor building cooling units (RBCU) have degraded heat transfer capabilities due to sediment accumulation from Lake Keowee. The low pressure service water system uses raw lake water to cool the LPI and RBCU coolers.

Duke Power Company analyzed under loss-of-coolant accident (LOCA) conditions the degraded perfomance of the LPI and RBCU coolers and concluded that the coolers would not provide adequate heat transfer to perfom their safety function. Therefore, Duke shutdown Oconee Unit 3 as it was starting up from its refueling outage and reduced power to 60% for Oconee Unit 2 and remained at 89%

for Oconee Unit 3.

The staff reviewed the justification for continued operation erd concluded that Duke did not show that the degraded performance of the LPI and RBCU coolers is acceptable. However, Duke provided detail on their analysis.

They stated that for the first 30 minutes after a LOCA, the RBCU and reactor building spray are not needed to mitigate peak pressure for containment integrity. This equipment is not needed during the 30 minutes, but only for decay heat removal.

The staff granted a temporary vaiver of compliance effective through Wednesday, April 8, 1987 until an emergency amendment to the Technical Specifications is issued.

I Indian Point Unit No. 2 On April 3,1987, Indian Point 2 experienced 'a secondary system pipe i'

break. The break consisted of a 3-inch " fish aiouth"on a 6-inch drain line from high pressure feedwater heater 26C..The licensee is currently evaluating the cause of the break.

The drain lines in the 2 other high pressure heaters were examined for degradation and were found to be acceptable. No injuries occurred.

NRC*iMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT FOR APRIL 6 - 12, 1987 1.

DEFUELING Pick and place fuel removal operations continued using a variety of tools. Some fuel removal was done vacuuming with the airlift tool.

Five canisters.were filled with debris. Three of the filled

ENVIRONMENTAL MONITORING I

US Environmental Protection Agency.(EPA) sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and the City of Lancaster sAgreement.

.'TMI water effluents are sampled from the station (Units 1 and 2) discharge and analyzed by EPA. Gamma spectrum analyses of the seven daily composited samples for March 21 - 28, 1987 indicated no TMI-2 related radioactivity, EPA's gamma spectrum analysis of the NRC's Tr1I outdoor air sample for April 2 - 9, 1987 showed no reactor related radioactivity.

The water works for the City of Lancaster composited seven daily samples from March 22 - 28, 1987. EPA's gamma spectrum analysis of the composite sample showed no reactor related radioactivity.

MEETING The Advisory Panel for the Decontamination of Three Mile Island Unit 2 will meet with the NRC Comissioners in Washington, DC on April 16, 1987 at 11:00 AM. The meeting will be open to the public and will include discussion of issues related to the processing and disposal of the accident-generated water.

OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest 1

Week Ending April 10, 1987 Babcock & Wilcox, Lynchburg Research Center Mr. A. F. Olsen, Senior License Administrator, Babcock & Wilcox Lynchburg Research Center (LRC) met with staff members of NMSS on April 6, 1987, to discuss the new LRC organization and its effects on the SNM-778 license renewal application under review by the staff.

The research center will now become the Naval Nuclear Fuel Division, NNFD Research Laboratory. The related changes as well as responses to NMSS comments will be submitted as page changes to the renewal application and will be incorporated in the final review of the renewal application.

Dry Spent Fuel Storage NMSS staff has completed its safety review of the General Nuclear Systems, Inc.

(GNSI) design of the borated stainless steel fuel basket to be employed in the GNSI CASTOR V/21 nodular cast iron cask (cask capacity, 21 PWR assemblies).

The staff has found that the borated stainless steel, Radionox A 18, has been adequately characterized by GNSI and that the design of the fuel basket meets the requirements of 10 CFR Part 72 and, ther: fore, is acceptable. With this determination, and subject to the conditions tf its materials license, SNM-2501, Virginia Electric and Power Company (VEPCO) mcy proceed to store spent fuel with an initial enrichment s 3.5 weight percent U-235 in CASTOR V/21 casks employing this borated stainless steel basket at its Surry Independent Spent Fuel Storage Installation (ISFSI).

To date VEPC0 has purchased 9 CASTOR V/21 casks.

One cask is storing fuel at DOE's Idaho National Engineering Laboratory.

Two casks are storing fuel at'the Surry ISFSI (i.e.

spent fuel with an initial enrichment 1 2.2 weight percent U-235 in casks empioying all stainless steel baskets).

VEPC0 plans to load 4 more casks at Surry this year.

IAEA Reverification at Turkey Point 4 The IAEA will conduct a complete verification of the nuclear material inventory at Turkey Point 4 from April 9 through 10, 1987.

Upon the successful completion of this inspection the U.S. will have satisfactorily reestablished international safeguards obligations at this facility as of April 10, 1987.

.NHSS licensing staff met with Dr. Marlin Remeley of Atomics International (AI) i on April 7 and 8 to discuss and define license amendment and security requirements for a potential seven year project involving remote decladding and repackaging of spent FFTF fuel (Pu0 mixed depleted UO ).

Presentations by US Ecology and California Department of Health Services e

Representatives from US Ecology, Inc., and California Department of Health Services (DHS) are scheduled to visit the Division of Low-Level Waste Management and Decommissioning on April 13-15, 1987.

Two presentations will be given on April 13 by US Ecology and DHS to inform Division staff of their progress in selecting a low-level radioactive waste disposal site in California.

Two working sessions will be held to discuss Division staff's comments on US Ecology's draft Site Characterization Plan on April 14. A wrap-up session is scheduled for April 15.

These meetings have been coordinated with OSP.

ITEMS OF INTEREST OFFICE OF INTERNATIONAL PROGRAMS WEEK ENDING APRIL 10, 1987 Foreign Visitors On Monday through Wednesday Mr. Jose Reig, Head of the Systems Licensing Division, and Mr. A. Munuera, Head of the PWP Systems Branch, Spanish Nuclear Safety Commission (CSN), met with ~ representatives of IP, NRR, and AEOD for discussions on various current nuclear safety topics. The visitors also. toured the Operations Center. Mr. Juan Unda of the CSN, who is currently in a long-term assignment to NRR, accompanied Messrs. Reig and Munuera. Mr. Reig indicated that the CSN has found interaction on regulatory safety matters with NRC to be very beneficial and that they plan to have technical interchanges with the NRC staff on a more frequent basis.

On Wednesday Professor Leonard Konstantinov, IAEA Deputy Director.

General and Head of the Department of Nuclear Energy and Safety, met with Chairman Zech, Commissioners Roberts, Asselstine, and Bernthal, RES Director Beckjord, NRR Director Denton, and IP Director Shea for safety discussions on matters of mutual interest.

Meeting with North Carolina Low-Level Radioactive Waste Management Subcommittee On April 6,1987, Kathleen Schneider of the OSP staff met with the North Carolina Low-Level, Radioactive Waste Management Subcommittee at the request of the North Carolina radiation control program. The subcommittee has the responsibility for drafting regulations for licensing a low-level waste disposal facility compatible to 10 CFR Part-

61. OSP staff discussed NRC's comments on North Carolina's draft regulation. North Carolina now plans to publish the revised draft regulations for public comment in May, 1987.

Coordination Meeting with State of Illinois Harold Denton, Director-designate of the Office of Governmental and Public Affairs and Carlton Kammerer, Director-designate of State, local and Indian Tribe Programs accompanied by Bert Davis, Administrator of Region III and Roland Lickus, Regional State Liaison Officer, Region III, visited the Illinois Department of Nuclear Safety <,nd toured their Springfield facilities on Thursday, April 9.

Included in the tour were the Illinois Radiological Emergency Assessment Center, the Radiological Assessment Field Vehicles, the Spent Fuel Inspection Vehicle and the Mobil Radiochenistry Laboratory. OFFICE FOR ANALYSIS AND EVALUATION OF OPERATIONAL DATA ITEMS OF INTEREST Week Ending April 10, 1987 1.

AEOD Case Study report C701, " Air Systems Problems at U.S. Light Water Reactors," was issued on March 31, 1987.

The study analyzes and evaluates the operational experience related to, and the safety implications associated with, failures and degradations of air systems at U.S. LWRs.

The report presents aspects of air system degradations and plant responses to air system losses which are not addressed in previous studies.

It also highlights more than two dozen events in which, contrary to licensing assumptions, a safety-related system failed due to an air system degradation or failure. Operating events involving the loss or degradation of air systems were judged to be safety significant because they may lead, under different circumstances, to potentially serious events and conditions which have not been analyzed in the FSAR.

The report presents five recommendations which, if implemented, should reduce reactor accident risks by reducing the likelihood for common mode failure of safety systems and by enhancing plant recovery from anticipated and unanticipated transients.

AE0D issued an Engineering Evaluation report on "RWCU System Automatic Isolation and Safety Considerations." Licensee Event Reports (LERs) for the period January 1984 through September 1986 and selected Daily Reports documenting events associated with the operation of the BWR reactor water cleanup (RWCU) system served as the basis for the report. The focus of the review was to:

(1) identify causes of frequent actuation of the RWCU system isolation valves by the RWCU leak detection system; (2) assess the safety significance of identified system failures; and (3) identify corrective measures that would reduce the frequency of the RWCU system isolation events. The report includes suggestions that the LER reporting requirements be reconsidered to eliminate the reporting of spurious, anconsequential isolation events and that the need for daily surveillance testing of the leak detection system temperature monitors be reconsidered in light of the low incidence of significant leakage events. Also, it was indicated that information in the report regarding RWCU system design and procedural improvements might be useful to the Institute for Nuclear 4

Power Operations in effecting excellence in operations and "as low as reasonable achievable" radiation control programs, for example, through its crgoing maintenance and new plant studies.

An Engineering Evaluation (AE0D/E706) on the " Mechanical Blocking of Valves" was issued in March 1987. The study reviewed 19 reported mechanical blocking events that occurred in a five-year period from 4

i October 1981 to February 1986. The investigation found that mechanical blocking devices were misapplied only to isolation or flow control valves.

The study concluded that the misapplication of mechanical blocking devices to isolation or control valves was caused by human error deficiencies, i.e., personnel errors or inadequate procedures. These events were not repetitive at any plant so licensee corrective actions were effective. Although the misapplication of mechanical blocking devices were infrequent, unrepetitive occurrences, a high proportion of the events could have and actually did result in significant compromises in safety. Accordingly, it was suggested that IE consider issuing an information notice to describe the underlying causes and actions that could be taken to minimize mechanical blocking problems. Also, because the misapplication of mechanical blocks was identified to be a human performance problem, the report was forwarded to NRR to determine if the maintenance and surveillance plan adequately addresses the deficiencies identified in the report.

An Engineering Evaluation report, documented in AE0D/E707, was initiated as the result of a report from Crystal River 3 in which design and construction errors resulted in a potential common mode failure of a system important to safety. A review of about 7,500 LERs received from January 1984 to September 1986 identified an additional 54 reports involving design and construction problems that pose a potential safety problem to plant operations.

' Essentially all of the 55 reports identified in the LER data base represent latent defects, that is, a design or construction deficiency was present but did not contribute to the occurrence of an event or of its severity. The deficiencies were not detected by plant quality control inspection or quality assurance programs during plant construction or modification, and in most cases could be detected through preoperational and startup test programs.

Reports were received from 34 plants, 21 of which started commercial operation in 1980 or earlier. Reports were received from 13 plants with less than 2 years of comrrercial operating experience. Of the 55 reports, 36 were errors in the original design or construction, and 19 were errors that occurred during modifications.

The design and construction deficiencies fell into six categories:

(1) piping stress greater than code limits; (2) incorrect hardware or improper installation of hardware;-(3) lack of fire seals for cable penetraction; (4) electrical wiring errors; (5) errors in electrical, instrumentationandcontrolcircuits;and(6)electricalandcontrol panels not seismically supported.

The deficiencies identified were attributed to inadequate control documents, unreviewed safety and design conditions, and inadequate review of modification.

About 0.7 percent of the LERs received by the NRC from January 1984 to September 1986 contained reports of design or construction deficiencies.

Furthermore, most of the deficiencies were discovered during reviews or inspectons initiated for a special purpose. Since not all operating reactors participated in the special reivews or inspections, the frequency of this type of problem may be underestimated by the statistics in this report.

The data point out the need for complete and well formed quality assurance and quality control programs to help verify that the plant as constructed is in compliance with design and construction requirements.

Commissioner Asselstine has approved the change related to vested pensions but disapproved narrowing the scope of the regulations as presently applied to fuel cycle activities.

  • Section 201 of the Energy Reorganization Act, 42 U.S.C. S5841, provides that action of the Commission shall be determined by a

" majority vote of the members present."

Commissioners Roberts and Carr were not present when this item was affirmed.

Accord-ingly, the formal votes of the Commission were 3-0 and 2-1 in favor of the decision.

Commissioners Roberts and Carr, however, had previously indicated that they would approve this paper and had they been present they would have affirmed their prior votes.
