Gentlemen:
SUBJECT: NRC INSPECTION REPORT NO. 50-302/86-13
The NRC Systematic Assessment of Licensee Performance (SALP) Board has completed
its periodic evaluation of your Crystal River facility.
This facility was
evaluated for the period November 1,1984, through March 31, 1986. The results
of the evaluation are documented in the enclosed SALP Board Report. This report
will be discussed with you at your offices in St. Petersburg, Florida on July 9,
1986, at 1:00 p.m.
The performance of your Crystal River facility was evaluated in the functional
areas of plant operations, radiological controls, maintenance, surveillance, fire
protection, emergency preparedness, security, outages, quality programs and
administrative controls affecting quality, licensing activities, and training
and qualification effectiveness.
The level of performance of your Crystal River facility was satisfactory.
Management involvement was evident in facility activities and indicated
appropriate attention to significant nuclear safety issues. The SALP Board noted
the continued strength in the area of maintenance. Weaknesses were identified
in the areas of security, quality programs and administrative controls affecting
quality, and training and qualification effectiveness. The repeated failure to
maintain vital area barrier integrity was noted as the most pressing security
issue requiring management attention.
The SALP Board rating in the quality
programs area was arrived at only after extensive deliberation. The declining
trend in the quality area was noted during the previous SALP period and has
become increasingly apparent during the current rating interval. FPC's apparent
inability to identify and correct its own problems was evident in both the
security and training areas.
Tne SALP Board noted that significant weaknesses
had been evident in the licensed operator requalification trainir
program, but
that performance in that area was improving. A declining trer.

was observed in the control of radiological work practices; inctc ed management
involvement and support are necessary in that functional area.
The SALP Board
expressed concerns regarding the recurrent problems with procedural inadequacies
and noncompliances and control of contractor activities; these problems were
prevalent across several functional areas.
