Inspection Report No. 50-293/86-07

This letter refers to the routine safety inspection (50-293/86-07) conducted by

Dr. M. McBride of this office on March 9, 1986 to April 28, 1986 at the Pilgrim

Nuclear Power Station, Plymouth, Massachusetts.

Dr. M. McBride discussed the

findings of this inspection with Mr. C. Mathis and other members of your staff

periodically during the inspection and at the conclusion of the inspection.

Areas examined during this inspection are described in the NRC Region I Inspec-

tion Report which is enclosed with this letter. Within these areas, the in-

spection consisted of selective examinations of procedures and representative

records, interviews with personnel, and observations by the inspector.

Within the scope of this inspection, no violations were observed. However, we

are concerned about two incidents in April where your staff did not take ag-

gressive corrective actions until prompted by the NRC.

In the first incident,

your staff's initial review of the debris found in the "2A" main steam isola-

tion valve was in our view superficial.

In the second incident, your staff

did not take action to correct a health physics contamination control problem

until after it occurred a second time.

In both cases, considerable NRC initi-

ative was required to assure that the concerns would be addressed. Recognizing

that these events occurred a few months ago, we believe that you should review

the implications of these incidents in light of your more recent organizational

changes.

In contrast to the above, activities associated with the reactor vessel instru-

ment line repair; including outage management planning and control, engineering

support, ALARA planning, and Operations Review Committee evaluations; were well

performed.

These differences in performance are illustrative of the inconsis-

tency in performance noted in the last SALP report.

We also reviewed your letter dated May 2,1986, in response to the inspection

results (50-293/86-06) transmitted in our letter dated April 2,1986. We ac-

knowledge the corrective actions documented in your letter; however, we are

concerned by what appears to be the excessive time period required to effect

changes in some of the areas.

Consequently, please be prepared to provide

an updated status of these items at the next management meeting currently

estimated for the week of July 28, 1986.
