NRC Inspection of the Trojan Nuclear Plant
This refers to the inspection conducted by Mr. R. C. Barr, Mr. J. L. Crews,
and Mr. G. Y. Suh of this office during the period of August 7 through October
1,1988, of activities authorized by NRC License No NPF-1, and to the
discussion of our findings held by Mr. Barr and Mr. Suh with Mr. Olmstead and
other members of your staff periodically during the inspection.
Areas examined during this inspection are described in the enclosed inspection
report. Within these areas, the inspection consisted of selective
examinations of procedures and representative records, interviews with
personnel and observations by the inspectors. Within the scope of this
inspection two apparent violatf or,s of NRC requirements were identified, as set

forth in the Notice of Violation, enclosed herewith as Appendix A.
Your response to this Notice is to be submitted in accordance with the
provisions of 10 CFR 2.201 as stated in Appendix A, Notice of Violation.
In
your response to Item B in the Notice of Violation, we request that you
address the measures that have been or are planned to be implemented to
improve the control of corrective maintenance activities that have a high risk
of challenging safety systems, including the quality and detail of work
procedures and instructions.
Several other issues which warrant your further attention were also
identified.
Specifically, the repiated failures of pressurizer pressure
transmitters (Paragraph 4 in the enclosed report), and the reactor trip during
flow transmitter venting (Paragraph 6) raise questions on tt.e implementation
and oversight of management expectations.
Additionally, the potential
problems related to inoperable, redundant safety systems during check valve
testing (Paragraph 5), and the problem with high radiation area access
requirements (Paragraph 8) give rise to questions regarding your staff's
understanding of ntelear safety fundamentals. We understand that you are
assessing these events, and would appreciate your evaluation be included with
in your response to the Notice of Violetion.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosure
will be placed in the NRC Public Document Room.
