  • APPENDIX A NOTICE OF VIOLATION TU Electric Dockets: 50-445/88-70 50-446/88-66 Comanche Peak Steam Electric Station Permits: CPPR-126 Units 1 and 2, Glen Rose, Texas CPPR-127 During an NRC inspection conducted on October 5 through November 1, 1988, one violation of NRC requirements was identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violation is listed below:

Criterion V of Appendix B to 10 CFR Part 50, as' implemented by Section 5.0, Revision 0, of the TU Electric Quality Assurance Manual, requires that activities.affecting quality shall be prescribed by and accomplished in accordance with documented i instructions, procedures, or drawings.

I Procedure NQI 3.09-M-005, "Field Verification of Structural and Miscellaneous Steel," Revison 7, requires verification of base metal damage / defects for loaded embedded plates. Specification 2323-SS-30, "Structural Dnbedmonts," Revision 3, Appendix A, Section 13, states, in part: "Damage / defect is the portion of the

! plate surface where the reduction of plate thickness . . . shall not exceed . . . 0.070 inch for 1 inch thick plate."

Contrary to the above, a gouge approximately 3/16-inch x 5/16-inch x 0.125-inch doop on the base metal of a loaded 1-inch thick embedded plate (PCRVP package CS-090-RB1-812-023-SO5) was not identified on Inspection Reports 1-0240944 or 1-0240742. The inspection was conducted to PCHVP Procedure NOI 3.09-M-005, l

Revision 7, "Field Verification Structural and Miscellaneous Steel."

This is a Severity Level IV violation (Supplement II)

violation: (1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when' full compliance will be achieved.
