APPENDIX A NOTICE OF VIOLATION Omaha Public Power District (OPPD)

OPR-40 During an NRC inspection conducted on October 24-28, 1988, a violation of NRC requirements was identified. The violation involved failure to control special processes.
In accordance with the "General Statement of Polic for NRC Enforcement Actions," 10 CFR Part 2. Appendix C (1988)y and Procedure
, the violation is listed below:
Criterion IX of Appendix B to 10 CFR Part 50 and the l wensee's approved quality asserance program description require that measures shall be established to assure that special processes, including welding and nondestructive testing, are controlled and accomplished in accordance with the applicable codes.
Subsection NB-4453 of the ASME Boiler and Pressure Vessel Code Section III,1983 Edition through Sumer 1984 Addenda, requires that unacceptable defects in welds shall be removed and the excavaticn area prepared for welding shall be examined by a liquid penetrant or magnetic particle method.
Contrary to the above, the repair instructions of Procedure Change No. 25571 to Modification MR-FC-81-51 did not include requirements for performing a liquid penetrant or magnetic particle exaraination of the a
excavation area prior to making a repair weld. The NRC inspector also observed repair welding being performed on welds F33, F45, and F144 of Drawing E-4097 Revision B and determined that the excavation areas were not examined by a liquid penetrant or magnetic particle examination prior to making the repair weld.
This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Omaha Public Power District is hereby required to submit a written statement or explanation to this Office within 30 days of the date of the letter transmitting this Notice.
This reply, should include for each violation:
(1) the reason for the violation if admitted. (2) the corrective steps that have been taken and the results achieved. (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full corrpliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
