SUBJECT:
TMI-1 STATUS REPORT FOR THE PERIOD MAY 30 - JUNE 6, 1986 Enclosed is the TMI-1 weekly status report from the NRC Resident Office at TMI-1 for the subject period. The NRC staff monitored routine plant operations at full power, and followed up on the June 2, 1986 reactor trip.
TMI-1 status reports are intended to provide NRC management and the public with highlights from an NRC regulatory perspective of TMI-1 activities for the pre-vious week. Subsequent inspection reports will address many of these topics in more detail.

ENCLOSURE TMI-1 STATUS REPORT FOR THE PERIOD MAY 30 - JUNE 6, 1986 1.
Plant Status As of 8:00 a.m. on June 6, 1986, TMI-1 was operating at 100 percent power and at normal reactor coolant' temperature (579 F) and pressure (2155 psig).

Facility Operations Summary The plant operated at 100 percent pcwer during the reporting period, ex-cept for approximately 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> on June 2-3, 1986, when the plant was in hot shutdown due to a turbine trip / reactor trip on June 2.

Items of Special Interest Reactor Trip i
The reactor tripped from 100 percent power on June 2,1986, at 2:11 p.m.
The trip was caused by a turbine trip due to loss of both electro-hydrau-lic control (EHC) pumps.
Plant maintenance personnel were replacino the feeder breaker to the IC turbine plant load center. The electrical lineup for this work had connected the loads on the 1C motor-control centers (MCCs) through cross-connect breakers to the ID MCCs. This resulted in higher than normal loads on a ID supply breaker, which tripped on over-current.
The licensee determined that the over current protection for one phase of the breaker was set too low. After repairs, the plant was re-started at 12:30 a.m. on June 3,1986, and 100 percent power was attained at approximately 1:00 p.m.
on June 3, 1986.
During the reactor trip, steam generator relief valves actuated, as expected. The associated re-lease of radioactivity was about 25 microcuries (i.e. 25 millionths of a curie).
Systematic Assessment of Licensee Performance (SALP)
The NRC Region I SALP Board meets 9:00 a.m., June 6 to assess licensee performance during September 16, 1985 to April 30, 1986. The SALP Board's written report issuance is expected in about a month. A SALP report as-sessing performance in major functional areas during only the restart period (September 16, 1985 - January 10, 1986) had previously been issued on March 17 and discussed at a public meeting in Middletown on April 24, 1986.

NRC Staff Status During the Period During this report period, the NRC staff on site consisted of the senior resident inspector and two resident inspectors.
The reactor projects section chief was on site June 5, 1986.

On June 6, 1986, NRC Region I issued inspection report 50-298/86-05, cover-ing March 7, 1986 to April 11, 1986. The report includes Notices of Vio-lations for two apparent violations of NRC requirements identified during the steam generator tube inspection outage.
In one instance there was a failure to properly survey for airborne activity, and this resulted in Reactor Building (RB) iodine concentrations going, undetected for up to fourteen hours, along with unplanned exposures to personnel, when the reactor coolant system was vented to the RB.
(This event is discussed in the March 21-28, 1986 Weekly Status Report.) In another instance, there was a failure to properly post a radiation area. Other outage activities, j
such as the isolation valve leak testing, and the actual eddy current testing of steam generator tubes, were generally found to be well-con-trolled. A written response to the report is expected from the licensee (GPUN) in thirty days.
